Please print and complete these forms before bringing them to our office before your scheduled appointment. This will help keep your waiting time to a minimum! During your first visit to Arlington Pediatric Dentistry, you'll be greeted by one of our friendly practice receptionists who will direct you to completing a medical history form and other required paperwork for your child.
May not bite, pinch, chew, suck or rub the "sleepy" area. Will experience an unsightly, ulcerated area if cheek, lip or tongue is bitten. It may look infected, but it is normal for the mouth to look this way when damaged. Do not apply numbing medication because it will allow your child to bite/reinjure the area.

This was our first visit and I couldn't be more happy! The entire staff is kind and friendly. I brought in my 3 year old and from beginning to end, the made us feel so comfortable and welcomed. They were talkative with my son and answered all his typical toddler questions. "What's that?", "What's this?" They were so great with his curiosity chattiness. We had such a great experience. My son already can't wait to go back!

We love this office! The staff makes my children feel like rockstars! Dr.Ketchel is so knowledgeable, patient and kind! She takes time with her patients! Today my child was in the worst mouth pain, Jo at the front desk was so helpful with my phone calls and when the pharmacy wouldn’t cooperate with my prescription, Dr Ketchel herself called them! I’ve never felt compelled to write a review but today was most definitely warranted! Thank you so much ladies!

Very impressed! Our assistant Britney was amazing with my boys. I didn’t catch the rest of the staffs names but from start to finish I had a great experience. I work in the dental field so I’m extra picky. However this team blew my expectations out the water. I was happy with their new covid measurements they are taking. It speaks volumes for how much the doctor cares about the safety of her patients and staff. I could easily see she had a well trained team and everyone enjoys working for her. I feel So thankful to have found this office.

Our first visit today was amazing....very clean, very professional, all staff was so friendly. They made sure to tell me exactly what it was they were going to do, listened to everything I had to say, told me what to expect in upcoming visits. I'm very pleased with this dentistry and hope to be with them for a long time

I took my three-year-old to the dentist office for his first visit and they were beyond fabulous. The facility was great, the kids had a lot of activities to keep them busy, and it was definitely state of the art. The hygienist helped us first and was awesome. The dentist was wonderful, talked to us for a long time, and gave me great tips and advise. They were great with my kids, one who wasn't even getting an exam but who they kept busy so that I could help the dentist with my son! I also really appreciated how the first exam was set up. They took us to a room that was really casual had a lot

This place exceeded my expectations greatly! I brought my 3 and a half year old and my 2 year old for their first ever dental appointments. The office staff was so friendly and patient. They let me do both of the appointments at the same time which was convenient and made my boys less nervous. The doctor and her assistant gave great advice and gave enough time for questions. There was no rush at all like how I feel at other dental offices as an adult. I definitely see why this office gets such great reviews - they live up to them!
