On behalf of our entire team, thank you for considering our nursing and rehabilitation center for you or your family member's care. Our newly remodeled facility features 58 skilled nursing beds and includes rehabilitation gym with state of the art equipment to help accelerate the recovery process of our residents.
We seek to maximize the quality of life for our long-term care population, who require a specialized restorative program. Of course, this service is included in the room and board fee. A positive and uplifting atmosphere is vital to the success of healing, recovery and rehabilitation. San Jose Healthcare & Wellness Center employs outgoing and friendly professional staff to create social programs and activities to fit the resident's needs.

The administration here is aloof and detached. The nursing staff are poorly trained and hardly communicative because of language barriers. During my short stay i was exposed to bacterial infections and i had an open picc line close to my heart. The port was left open overnight on more than one occasion. I was given a commode to use that had dried feces from other patients here. I was given rotten milk and lukewarm tap water to dri k. The response by staff for call button assistance was delayed and non existent at times. The building is dated run down and dirty. I would not recommend this facility

My mom was transferred there from O'Conner Hospital after her hip surgery. I couldn't go inside because of covid-- but I insisted on seeing her every day through the sliding doors.
I was told they had extra staff for Alzheimer's patients. Nope.
Actually, the mixed up communication in this place is tragic.Don't expect regular updates! The nurses act really nice-- and some truly are-- but they just ignore/ don't hear/ too busy to answer my mom's yells. My mom couldn't use the call button. I had to stand outside and call in for help ..and many times none would answer. When they had a covid outbreak

I did not want to go to a nursing home but I am so glad I did they have educated me allowed my voice to be heard and have taken wholehearted care of me I couldn't have been more blessed for all the staff and all their support. The social worker and case Manager are excellent quick to solve any situations I had or others were going through. She insured me about medicines, my diet and home therapy Therapy was very informative and ensures my safety the whole time. I will refer anyone here because they have quality care

I wish I could put a negative. My father was placed here after neck fusion surgery. Pretty major and painful surgery of the front and back of his neck. He was sent there for rehab. He was handled roughly causing him to scream in pain only to be told they didn't know he had neck surgery.. but the worst was not answering his calls and leaving him in his urine all night.. stripping him of his dignity. My step mom picked him up immediately when he called her. They said they were short staffed. I pray for any of the poor victims the that have no advocate or voice. And shame on the horrible people associated

The Director of social services was rude and unprofessional she approached a friend who had his mom there for dementia and rehab the women yelled out in fromt of viitors. " your moms not going home shes staying here permanently". And a secondary visit she came to meet with him and said i told.you "shes not.going home shes going to die here " the poor lady juzt lied flat down and.played like.she was sleeping how sad is that.

Please do mot take your loved ones there. The staff is incompetent, rude and careless about their patients. While my mother there she was ignored when she needed to be change, she continuously press the button for assistance and was ignored. I observed patients sitting in the hall way left unattended for hours. The place smelled like urine. There was a few members of the staff that tried , but were unable to serve all of their patients. These workers need to obtain a different job in a different facility, but they stay because they feel bad for the patients there.

We have a lot of family in the San Jose Area, so when my 89 year old uncle needed long term care, due to his dementia, this was the perfect place for him. Even with his limited faculties, the staff was able to engage him in many activities, including preparing food for the homeless!. These social pursuits are imperative to his maintaining a quality of life that he would never be able to experience on his own. We are so appreciative to everyone here. My cousins visit daily and are extremely satisfied with the care he is receiving. This place is welcoming, warm and wonderful.

Poor & not attentive to patient needs assistance . I have a nightmare experienced about this place. A nurse called me @ 5 am claiming she cannot get any vitals on my closed relative , as fast as I arrived within a minutes , Oh my God , he's so cold & stiffed , thats totally negligence , but God knows what goes around comes around for sure , KARMA WILL DO THE REST!
