We understand that arriving at a doctors office for the first time can be nerve racking. Filling out paperwork, learning new faces and names, and getting to know your way around the office can be an overwhelming experience. That's why at Wells Family Chiropractic, PLLC our aim is to remove any stress or unnecessary tension and to make you feel comfortable.
On your first visit you will need a patient privacy form, photo release form, informed consent, waiver and the appropriate questionnaire. This paperwork will provide us with your health history and information on your condition. This paperwork will provide us with the health history and information of your childs condition.
Chiropractic care is like building a house - certain things have to happen in a particular order in order for everything to stand strong and work correctly. When building a house, if you tried to put up your walls before you had a solid foundation, your walls would be weak and eventually collapse. If you tried to put on your roof before the walls were ready, you would run into the same problems.
Upon entering our office, one of our Chiropractic Assistants will welcome you and get you started right away. At this time we will request that you complete our new patient forms if you have not done so already. The new patient forms required are avaiable in the New Patient Center of our website; if you can print and fill out prior to your visit it will save you some time.

My experience at Wells Family Chiropractic has been very rewarding, both physically and mentally. They treat the entire body and truly care about their patients well being! Dr Chad and his assistant Jessica are uplifting, kind and very personable. I highly recommend Dr Chad. His treatments have helped me a great deal. My spine is stable and my pain has been reduced!! He has definitely improved my quality of life.

Dr. Chad is such an amazing chiropractor! He is extremely thorough and truly cares about getting your spine back into a healthy alignment! My family has seen so many health improvements with continuing with our regular chiropractic care for the last 8 years. We are so lucky to have been blessed with such an amazing chiropractor!
