Services include: Psychosocial Rehabilitative case management, therapy, medication assessment/ monitoring hospital discharge planning, 24/7 crisis line, and transportation. Our staff include certified case managers and licensed therapists ( LCSW, LPC, LMFT) all who have had extensive training and experience in meeting needs of consumers with Severe and Persistent Mentally Ill ( SPMI) adults in their home/ community setting. We partner with community providers, MD's and Nurse Practitioners, for medication assessment and monitoring then follow up with in-home nursing care.

With the expertise of our licensed therapists (LCSW, LPC, LMFT) we utilize therapy interventions that are designed to meet the consumers individual needs.
National Smart Healthcare Services, Inc. will conduct a review of your hospital treatment. We then collaborate with the hospital team to design a discharge plan that will ensure safe home transition and re-integration into the community. Transportation to home is provided for some hospitals. The goal of this service is to ensure that the patient has uninterrupted access to support and health services as they leave the hospital for home.
