Every job-related injury should be reported as soon as possible to your supervisor. Injury also means any illness or disease that is caused or aggravated by the employment as well as damage to medical braces, artificial limbs and other prosthetic devices. Before you obtain medical treatment, ask your supervisor to authorize medical treatment by use of form CA-16.
In some ways, one could say that chiropractic care is a bit like building a house - certain things have to happen in a particular order in order for everything to stand strong and work together correctly. The staff is great at times you don’t feel like you at therapy that you’re with family They make sure that you’re not hurting when you’re doing your therapy and if you feel like that is it helping the area that is hurting they will try something different to make sure that you get the best treatment When they released me from therapy I think I’m gonna feel like I lost a love one😢

I have been going to North Florida Medical baymeadows location for over 2 years now. I love the one stop shop for all your needs. The doctors have been great at taking care of me, and referring me out where needed. The front desk staff and Rainey the operations manager is always friendly and helpful, even when I'm not because of the pain I'm in. Chiropractors are great and have different ways to address your needs. Physical therapy staff (Stan, Chris, Dennis, Barbara) is very knowledgable and always in an upbeat mood to help motivate you. I went to see Mr. Otto in the Orange Park office and my initial appointment was a disappointment because the mechanism he used did not alleviate any pain. I called an hour after the appointment and left a message explaining I wasn’t happy and requested if I could see another chiropractor. No response. I emailed the main… office a couple of days later and explain what happened and that I was upset I went IN in pain and left with the same pain. I was invited to the main office to see another chiropractor.
To the new person I explained my expectations and what
