We are dedicated to supporting our patients 7 days a week. Pathways Healthcare has built a triage team to meet the needs of patients 24/7. Unlike traditional visiting nurse agencies, Pathways has a team of physicians and nurse practitioners available 24/7 for rapid medical intervention, which prevents unnecessary emergency room visits and re-hospitalizations.
Home health care is a wide range of health care services that can be provided in the comfort of your home for an illness or injury. Pathways Healthcare offers an innovative model committed to supporting the medically complex patient. To provide this higher level of care we have physicians and nurse practitioners on our staff who can provide rapid medical intervention.
A program designed for hospitals that require urgent in-home medical assessments of patients who have had an exacerbation of an existing disease or injury. The Post intensive care team is driven by Physicians and Nurse Practitioners who provide home visits until the patient has been stabilized. Our innovative model is perfect for home health patients because it adds the unique feature of a physician organization.
Palliative care is designed to provide relief from symptoms from serious illnesses and side effects of treatments. Patients can receive palliative care while working towards a cure or transitioning to end of life care. The diagnosis of a serious illness does not always require hospice services. Provides relief from pain and other distressing symptoms (physical, psychosocial, and spiritual).

I had a serious injury and Pathways was referred to me by the hospital. Intake and discharge were handled professionally and promptly. PT and OT visits were scheduled a week in advance and the time was confirmed each morning of a scheduled visit. The three people who visited me were prompt, professional and interested in how they could improve my day-to-day ordeals. I am so grateful to have had that in-home support -- it would have been a struggle without them. I highly recommend them.

Why would they take on new clients and they can't keep not one appointment? Never shows up for any appointments, that they scheduled. Never shows up for important iv medicine or iv changes. Supervisor Tiffany has no patient etiquette. Never returns a call. Never reaches out to client or clients family. VERY, VERY DISSATISFIED!!! The best visiting nurses but management, scheduling, the list goes on very unprofessional. DO NOT USE THEM. NOT A GOOD COMPANY

My home health nurse was useless. the physical therapist set me back in my progress which had been constant until i started seeing her. she preferred to gossip and give "information" about health - i found her to be a very UNintelligent person. i was not interested in listening to her. i wanted therapy to get better. I THINK THESE PERSONS WASTED MY TIME AND MY INSURANCE COMPANY'S MONEY.

Pathways Healthcare took care of my father after both of his hip replacements. He went straight home from the hospital and recovered better than he ever was. He is now up on roofs and climbing ladders again! His care team from Pathways was INCREDIBLE and he swears they are the only reason he had the will to recover.

Joe the OT and Steve the PT at Pathways were amazing in taking care of my 94 year old Dad. Both have such great knowledge in OT/PT and handled my Dad with such kindness. My Dad gained a lot of strength and mobility after a few weeks of work with them. His PCP was very impressed with the progress. They were professional, caring and very patient. Fantastic and just awesome people! Thank you!

I cannot say enough good things and glow about Pathways Healthcare. During our introduction they stated that they founded the company on the principle of bringing family oriented healthcare into the home. My Father-in-law has had 2 strokes in November of 2016 and March of 2018. Pathways was recommended as they offered a variety of services from Home Nursing to Physical, Occupational, and Speech Therapy. All, and I mean all, staff were professional, courteous and caring. Phone calls to arrange that day's meeting times were made in the morning. Questions were answered and next visits times were planned

Where do I even begin.... They are not even worthy of one star. If they even show up they are useless. My family and I were shocked. They constantly would give a time to be at the house to administer IV meds and then say they couldn't make it. They would ask if we could do a different day because they were no longer in the area. Their schedule had apparently changed. Meanwhile, this family member vitally needed her meds at a certain time every day. They told us that wasn't always possible because they only work 8-4. I cannot day enough great things about Pathways! They are a class of their own! While providing the typical visiting nurse services and therapy, they also have doctors and nurse practitioners on staff for their patients as well! Staff are courteous and knowledgeable. I would not trust my loved ones with any other homecare company!!

Changing homecare! Pathways is made up of the most diligent and hard working clinical staff I have ever encountered. These are not just names on a chart like to most doctors; each clinician cares for and about every patient they interact with. Homecare is not an easy field, but Pathways does it right!

Stay away from Pathways because they promise you everything and provide the worse possible service. They never return your phone calls, do not have the proper staffing to take care of their clients, cause more harm than good and are dangerous to the health of their patients. They have a salesperson entice you to use them who also promises the best service and providers and then you never hear from them again.

While the therapists and nurse were exceptional, the nurse practitioner was incompetent and deceptive. She convinced my 24/7 aide to ask for time off, said she cover for him, then never showed up. Without authorization, she contacted my physicians, told my PCP his services were not needed, and continued to act even after I changed from Pathways to Partners. Add in losing other patients records and leaving then unattended..If you're looking for home care, I strongly recommend you look elsewhere.
