Home Health Services - Under a Physician's order, the Home Life Healthcare Team can provide one or more of the following services:. Our Chronic Care Management Program from Home Life Healthcare helps patients with chronic illnesses - such as diabetes, heart failure and breathing problems - to better manage their health and achieve the best quality of life possible.
Incorporated in 2007, Home Life Healthcare has been serving homebound and chronically ill Medicare and Medicaid patients after obtaining a skilled care license with the Illinois Department of Health in 2008. In 2010, Home Life was initially accredited with The Joint Commission and was granted Medicare Licensure.

TERRIBLE COMPANY. DO NOT GO THROUGH THIS COMPANY. The employers do not care about their clients or their employees. The care plans rarely ever match the clients needs and the case workers never care to look in to what care is actually needed. They just send employees out and only care about receiving a check. They do not care about their employees safety whatsoever. They send them into dangerous environments. They need to do more thorough background checks on people who are staying in the clients homes. TERRIBLE SERVICE AND TERRIBLE COMPANY. The whole company should be investigated and shut down
