The Family Practice and Counseling Network (FPCN) provides person-centered, integrated and comprehensive health services to individuals and families across their lifespan. The Network promotes resiliency and well-being among patients, staff and surrounding communities.

Utilizing a multidisciplinary approach, FPCN provides a variety of services under one roof, including primary care, behavioral health, dental, cardiology, podiatry, diabetic education, prenatal care, physical therapy, family planning, a pharmacy, and more.
Services
Family Practice and Counseling Network opened with an initial Health Resources and Services Administration (HRSA) grant in 1992, and has since grown and developed in response to need and demand by the communities served.

The Network began with one converted apartment space within the Abbottsford public housing development, and now consists of three comprehensive health centers in Philadelphia - Abbottsford-Falls Family Practice & Counseling in the Northwest, the Stephen and Sandra Sheller 11th Street Family Health Services of Drexel University in the North, and the Health Annex in the Southwest; a convenient care clinic in Philadelphia - Fast Family Care @ Shoprite; and a Philadelphia site that is a partnership with Action Wellness, called Annex West, located in West Philadelphia.
FPCN's Nurse Practitioners and other Primary Care staff provide patient-centered care, including medical care for pediatrics, adolescents, adults, and older adults. Well child exams, physicals, immunizations, health screenings and family planning are available for all patients, as well as gynecology and prenatal care for women.
Virtual visits are available to Behavioral Health patients so that you don't have to come into the office during the national emergency. These Virtual visits may be over the phone or through a video call, depending on your needs and availability. Current patients, who need a virtual therapy appointment scheduled, please call your Behavioral Health therapist directly.
Family Practice and Counseling Network, a program of Resources for Human Development, is a network of federally qualified community health centers that uses a nurse-managed, integrated model to deliver primary care, behavioral health, dental, and preventive services, which are co-located at the centers and are designed to treat the whole patient.
FPCN is committed to the creation of a trauma-informed system of care that continually recognizes and responds to the impact of traumatic stress on all those who have contact with the organization, including children, adults, families, caregivers, and staff. FPCN seeks to infuse and sustain trauma awareness, knowledge and skills through every aspect of the organizational culture.
