Please call our office (928)771-1700 for a courtesy insurance check for physical therapy benefits for your particular plan. If you do not see your insurance company listed, please call our office. Many insurance companies are part of larger networks that we may be contracted with. Some insurance policies have only minimal differences between in-network and out-of-network benefits.
