It is about 8 pages that cover your medical history including prescription medicines, accidents, surgeries and previous doctors. If you do not complete in advance, please bring this information and arrive about 30 minutes early to complete in the office. My allergies made my life very miserable. I was extremely congested most of the time and had much difficulty breathing.

Dr Gannt is so full of himself.
I had pretty much the same experience that another reviewer had:
We talked on the phone, he noticed my Spanish accent and proceeded to "speak in Spanish". I'm using quotation marks because he does NOT speak Spanish, not even a little bit.
I kept speaking in English and he kept trying to dazzle me with his inexistent Spanish skills. He thought he was being charming and helpful (poor immigrant me), but he was just being racist.
Not only am I Hispanic, but also a woman. So to top it all off, he treated me like a clueless dumb girl who needed parenting

Wow. I just cannot believe the ability this doctor has. You have to go w an open mind. His method will seem like voodoo or as if he's pulling a bad prank on you but give it time and you'll be blown away. Lol
I was allergic to chicken, rice, beef, pork and even lamb which I love so much. That's not all. Lettuce, tomatoes... there goes my salad.
The other allergist told me to avoid all these foods for the rest of your life or else you will always be miserable after eating them.
These foods would give me lots of acid reflux, gas, heart burn, stomach pain or I would break out badly

I called to ask if the provided sport physicals and the guy didn't understand me so he asked me if I speak Spanish I said yes so he tried talking in Spanish and I told him i can speak English more than fine and I told him that and he kept on talking struggling to speak in Spanish and then he said just cause I'm Hispanic he can give me a physical for 60$ and I told him it's fine I'll go somewhere else and he answered poorly making fun of me and Hispanics. Horrible phone service
