As a service of Urbandale Health Care Center, our TruRehab therapy program offers unique rehabilitation services to all of our Guests. We offer physical, occupational and speech therapies with our own full-time staff. Specific therapy services are based on the initial clinical assessment of each TruRehab Guest.

To supplement therapy sessions, our TruRehab nurses and CNA's are trained to assist our Guests with activities of daily living and provide ongoing education that will benefit our Guest when they return home. Other unique features of our TruRehab program include private suites, complimentary local telephone services, flat-screen televisions, Cable TV services, and private dining and lounge areas.

If you have questions or concerns that you feel are not resolved or answered appropriately, please ask to speak with our Administrator, Bethany Lee or Director of Nursing, Nikki Ascherl.
By choosing Urbandale Health Care Center for long term care, we understand that residents and families are placing their trust in us to provide quality care and customer service. Our dedicated medical team provides a full spectrum of nursing and specialty services 24 hours a day.

Our professional and caring staff is available to provide personal care for those residents requiring daily living assistance or complex skilled nursing treatment that is based on the individual resident's needs.Medicaid is a Federal and State-funded program operated by the Department of Human Services that pays necessary medical expenses for many low-income individuals.
Knowing that a loved one is suffering from a debilitating illness or possibly even nearing the end of their life, it is often very difficult to deal with mentally, physically and spiritually. The staff of Urbandale Health Care Center combines knowledge and empathy to help manage a patient's symptoms and make the patient as comfortable as possible.
Respite care is designed to relieve individuals or families involved in caring for a loved one who is no longer physically or mentally capable of taking care of themselves on a daily basis. Urbandale Health Care Center offers a respite care stay program that allows for caregivers to take a break and take care of themselves.
We are committed to always doing the right thing. That is why we have an ethics and compliance program and why we have adopted the Code of Business Conduct and Ethics. The Code is specifically designed to be part of an effective program to prevent and detect violations of law and conduct. Employees are encouraged to discuss any questions related to this Code with their supervisor or the Corporate Compliance Officer.

Staff was not friendly at all. Very disappointed that my brother has to be there!!! Hoping that their week day staff is more caring then their weekend staff!!!!!
I wouldn’t recommend this place to anyone!!

Kind of a mixed bag. Great care as long as Management is in the building, one-star at best when they leave for the day. Lazy, gossiping CNAs who blame the residents for their incompetence, third-shift nurse who berates patients for reporting incompetence, your family member will lay in a soiled brief for HOURS after management leaves. This group’s facilities in Newton and Altoona are even worse. If your family member is bariatric they will be treated horribly after Management leaves for the day. Don’t get me started on the Weekends….

My mother is a current resident there. She fell during the middle of the night and after she used her emergency button they took an hour before they have responded. She has had things stolen from her room. My mother is beside herself and I am trying to get her out but I dont know where she would go.

I used to work there. A nurse "J" was a rude one. She told the residents to "be patient " when they were asking for Tylenol. The Assistant Director of Nursing who used to work there "S" would be at the nurses station talking about her personal problems to her best friend nurse but expected everyone else to work hard. Hopefully there is a new Assistant Director of Nursing there now that works harder. The PRO is that they have good food but is is freezing there in the winter.

This is the worst place to be employed at. Management doesn’t help with anything. Majority of the staff is rude and don’t care about on another. Very short staffed and only 1 housekeeper so rooms and hallways are filthy. The poor residents have to live with roaches because there’s no proper cleaning being done. And they don’t sanitize anything in the building. Please do not send your loved ones here! The entire building is 83 degrees all day and night. and in the winter time the heaters don’t work.

There are staff members who have been there a long time, however, there is turnover all the time of nurses and aides. The turnaround could be due to their working real hard and being short staffed. Then, leads to staff being rude to patients and family members. Also, my mother 's hair wasn't combed several times on her shower day when I went to visit; her space wasn't clean (I swept her floor with kleenex). When I was cleaning my mother's floor with kleenex, I found her crocks under recliner. The Social Worker tried to tell me the staff found them under her bed. No, I found the crocks and set them

My mother-in-law has stayed here twice after hospitalizations. She has received excellent care and rehabilitation. The rehab rooms are private rooms with phones, etc. Her physical, occupational, speech therapy, etc. were very professional and there is a social worker who helped plan her discharge. She made friends with the other patients and enjoyed the socialization very much.

This place is amazing! If you are looking for care for an elderly loved one please consider this place. It should be noted that I was SO skeptical considering even touring but the hospital was adamant we needed to give it a chance and I'm SO happy we did. It's important to note that this is NOT the same place or under the same ownership it was under more than 3 years ago. From our tour to when mom went home everything and everyone was superb. The administrator was even involved on a day to day basis with moms care and heard and attended to any and all questions we had through the whole process
