The environment in which people live can truly affect their mental and emotional wellbeing. We partner with agencies to help them adopt the "pathways hub model"-the same model HCAN uses to serve our clients. Our Maternal & Child Health Care Coordination assists moms during and after pregnancy to ensure they have a full-term baby and that mom stays healthy.

It helps in many ways, especially through education and addressing the needs of the expectant moms. Our Adult Health Care Coordination sends a community health worker directly to an individual's front door. We assist clients in taking away the barriers that would otherwise prevent individuals from getting routine care and managing their condition.
Services
Health Care Access Now (HCAN) was started by community leaders in 2009 to improve the overall health status of Greater Cincinnati's most vulnerable residents - those who are likely to have poor health outcomes because they do not have consistent medical care; access to behavioral health care and specialty care; lack support in successfully managing chronic diseases.
Adult Health Care Coordination connect our Community Care Coordinators (Community Health Workers) with patients from primary care practices and others who are referred from Medicaid health plans. The Coordinators complete a social and medical access assessment to identify the needs of patients in meeting chronic disease self-management goals as well as finding resources to eliminate barriers to regular and timely medical care.
Education, transportation, child care - barriers to these services (and more) can keep an individual from getting the support they need to get and stay healthy. Community based Care Coordination pairs individual, diverse needs with the complicated matrix of services in our community. Care Coordination services are provided by a specialized workforce - Community Health Workers.
Adult Health Care Coordination connects our Community Health Workers with patients from primary care practices and Medicaid health plans. The CHWs complete a social and medical access assessment to identify the needs of the patient. Together they craft a plan to meet chronic disease self-management goals, which include finding resources to eliminate barriers to regular and timely medical care.
Our goal is to ensure that every woman will have a healthy pregnancy, delivery and ongoing medical care. Women in our program are referred by Medicaid health plans, physicians, United Way 211 and also self-referrals. Each woman is paired with a Community Health Worker who provides important education about breastfeeding, safe sleep and assists with smoking cessation resources.
