Services
We operate Federally Qualified Health Centers (FQHCs) in Bay, Calhoun, Franklin, Gadsden, Gulf, Holmes, Jackson, Liberty, Walton and Washington counties to primarily serve people on Medicaid, Medicare, and those who are uninsured on a sliding scale. Our Community Health Centers provide primary and urgent care, as well as dental care and mental health support.
New patient paperwork submitted to our mobile clinic programs will be kept on file until the event date. Mobile clinic events are not operated on a scheduled appointment therefore, you will not be contacted to schedule an appointment. Patients are seen on a first come, first serve basis at all mobile events.
Select the English or Spanish Version. Once you've completed and submitted your application, the clinic will contact you to schedule your appointment within 24 business hours. Forms can be taken to the location you wish to be seen at or they can be submitted via fax. Fax numbers are available under Locations, then select your desired clinic location.
A Patient-Centered Medical Home is a system of care in which a team of health professionals work together to provide all of your health care needs. We use a team approach to communicate and coordinate your care and provide the best possible outcomes for you. As a patient, you are the most important part of a patient-centered medical home.
All full-time permanent positions include 13 paid holidays per year plus paid time off (PTO) accrual, health, dental, vision & life benefits, excellent retirement savings plan. We currently have a total of 14 clinics and 10 mobile clinics, with more locations planned. As we continue to grow, we need more talented team members to fill the following positions.

After having an appointment for three months I arrived at the clinic to find my appointment had been given to a cash paying customer, I had insurance. They used the excuse that it was a computer error yet they wouldn't cancel his appointment and see me. The only thing they could offer was to say they were sorry and I would just have to reschedule. My last appointment the dentist there had to send me to a real dentist to tell them if they could cap my teeth or not. This was at the Freeport office. Then to add insult to injury they sent me a letter saying they would no longer see me because I called
