Urethane mortar concrete has a wide range of applications that can be applied to a range of substrates like integral cove bases, slopes towards drains, and other inclined surfaces. Forms Designs are OUTSTANDING. They squeezed me in, came when they said they would, did fantastic quality work in a timely fashion and at the price they said they would. Very professional, courteous, friendly, and very knowledgeable and helpful at deciding what to do, color of floor, pluses and minuses of different alternatives. Excellent quality and I cannot say enough good things! I highly recommend them.

My garage floor was a disaster. I had a crack in the floor from settling that popped up the linoleum tiles on it. I pulled all of the tiles off and realized that I had to epoxy the floor as that seemed to have the best results on durability.
I contacted a few flooring companies and Forms Designs came out for an estimate that day. He was professional and detailed. I made the decision to schedule the following day and they were able to come out within two weeks. The floor was completed in the same day.

Marco and his team were amazing. The Garage is 1000% better and the quality and watching the guys work and the tiniest spots Marco sees and makes adjustments that the normal human wouldn't see. Also seamless process as they moved all the cabinets and Freezer and put back. A++++ If you are questioning whether to use them I wouldn't and our floor was put down while it is was 20 F.

Marco and the team at Forms did an outstanding job w my garage floor. There were areas where crumbled concrete needed to be removed and repaired. They took two full days and left their work area clean and my garage now looks like new. Great communication and competitive pricing.

I can't express just how much I love my new garage floors. Forms Designs is top notch. Marco was great. He explained the process and the time line clearly and my experience was exactly how he described. Once we decided to go wth Forms Designs, Marco came over to talk though our color choices. The customer service was world class. Forms Designs is a small company that takes customer service seriously. Outstanding job on my garage floor. The installation crew were extremely knowledgeable, courteous, neat and are true professionals. I never realized my garage floor could look this spectacular.
