Our Activities Program is full of fun, informative, and entertaining events for everyone. Each day there are plenty of activities to not only keep you busy, but you will find it is a great place where you can go to socialize with others. Join us for ice cream socials, arts and crafts classes, music appreciation, current events, or a lecture series.
Falls City Nursing and Rehabilitation Center is a modern, state-of-the-art skilled nursing facility, conveniently located in a quiet neighborhood on the eastern edge of town. We are at the corner of E 17th Street and Burton Drive. We feature 63 beds and beautiful spacious rooms in either private or semi-private settings.

If you have complaints about this place they will give your family member a 30 days notice to get out, my family got a bed wound that they did nothing about they refuse to take or set up doctor appointments they do have supplies the list goes on, So short of staff agencies staff don’t want to work there they said in all the places they go this place is the worse. Shannon the DON will refuse to speak with you and Spencer won’t return calls, Don’t send your family here if you truly love them
