Grace Compassion Home Health (Gradark Compassion Care, Inc.) is proud to be accredited by CHAP, (Community Health Accreditation Program) the independent, nonprofit, accrediting body for community-based health care organizations. We consider many factors in determining how to fulfill the needs of each individual patient. We begin with a thorough review of the physician's report, conduct our own patient and environment assessment, and then formulate a plan of care which is evaluated monthly and update as needed to accommodate the patient's progress.
Under the direction of a physician, our nurses will conduct a patient and environment assessment, and coordinate your care planning with all team members. They will provide treatment and educate patients and their caregivers on the disease or injury process, treatment plan, medications, diet and wound care.

I have never worked with this kind of terrible agency before they are somehow fraudster they dont ever want to pay for the service u rendered nd when u ask for ur money too much they use it as an excuse of fering u which is very bad, even most of the client the have are aggressive too very poor and bad agency to work with pls dont fall a victim do not work for them am still expecting my last check anyway

When members of our community do not receive the services or open lines of communication, it is troublesome. In my profession, if we do not respond to client inquiries, we get reported to the state regulatory board's. Apparently, this entity is not concerned. Our family member needs answers and we need them immediately before official complaints are registered. Too many government contractors like to get the money, but don't like to do the work required to maintain the contract.

This is not a home health care service that accomadates their clients. The owner does not return phone calls to the clients or their family members. My family member has experienced, attendants wanting to be on their cell phones, not being sanitary, rudeness and lack of hours fulfilled. Miss Grace (owner) wants assigned clients to accommodate her… aides needs over the clients. Miss Chundra is an office staff member that I give kudos for stepping out and extending service beyond her job title.

I just love the nurses at grace compassion home health, they have never let me down, and compassion may be in their name but they really do show compassion and put my family members needs first. The nurse for my family member is great shes always on time, and the nurse who have covered are so pleasant and sweet, for the time my family member has been on services with them they have never let us down, they met every need we had and my family member made a tremendous recover and we are so appreciative . They really do care for their patients

My grandma was on services for a good couple months and let me tell you when i say grace compassion made a great impact and had her improve drastically, they provided her with physical and occupational therapy when she needed it, they helped her get her oxygen and medical supplies she needed. They even sent a house call doctor so she didn't have to find rides to her appointments. Her nurse was wonderful and and always handled our concerns. And the aide was great she helped keep everything clean. Im very thankful for the grace compassion team and helping my grandma make her health grow, she benefitted
