Services
Admissions are coordinated by our Administrator, Admissions Coordinator, Director of Nursing and Social Services Director. All referrals and inquires are reviewed by our team. Once the necessary medical assessments and financial information is received, a prompt response on appropriateness for placement can be given.
The quality care we provide is accomplished by staff that is trained to care for the individual needs of each resident. We provide care according to each resident's particular functioning level, ranging from early dementia to medically complex needs. A Licensed Registered Dietitian conducts ongoing assessments of each resident's nutritional status.

Unfortunately I have been given the opportunity to visit this Rehabilitation and long term center. Very nice and clean. Staffing for this center is fantastic. Rehabilitation area is the best in the Temple area. Staff readily available a assisting all needs of patient. 5 🌟's👍 is what I have to give. I would give more if I could. I highly recommend this center 👌 for Rehab and/or long term.

This is a very nice and modern facility. My grandmother has stayed there a couple of times. This is where she chooses to go for rehab. The only issue I have is that the weekend staff seems to be lazy. You come in during the week and everything's taken care of but when you come in on the weekend it seems like it is a different crew. They told me that my grandmother could not get out of bed to visit. This is ridiculous. She is capable. I felt like they just did not want to deal with helping her get up.

My mother was in here awaiting heart surgery. After having an angiogram, the PT put her on a mechanical bike and she developed an 18"l x 5"w x 2 1/2" h in her groin area. Put her heart TAVR surgery off 2 months. Surgeons were mad. She also was in congenitve heart failure and dr said she had pneumonia. On night my mom couldn't breathe and the in call dr could not be reached by Cornerstone nor S&W page bc she lived in the country (was told this is not the first time this has happened). Charge nurse was rude also! Nurse Jennifer does not need to be working there. We have paid our bill and left in
