There are varying degrees of bipolar conditions, from milder forms with depressive feelings and occasional higher level energy.
It had been a week of suffering from anxiety and panic attacks without telling anyone. I was trying to deal with it on my own. After a week, I couldn't handle it anymore and I was so scared! Deborah agreed to meet with me when she usually does not have office hours, which I was so grateful for because I needed help and she was there for me.

This section of the Notice of Privacy Practices is to be removed from the rest of the pamphlet, signed by the individual and placed in their chart or record to be retained for a minimum period of 6 years or for the length of time designated to retain patient records. Should a patient refuse to sign the acknowledgement, a note to that effect must be recorded in the individual's chart or record.
Please be advised that we are "in-network" with federal Medicare only, but will bill your primary insurance company as a courtesy and accept "out-of-network" benefits providing you have them. In some cases, we accept assignment of benefits but in all cases, we require that the guarantor, the person who is financially responsible, is personally liable for any balance not covered by insurance.
