Our team of highly trained rehabilitation and nursing staff are focused on short-term transitional goals for your successful return home, as well as long-term quality of life needs for permanent placement.
Our highly skilled rehabilitation staff includes dedicated Physical, Occupational, and Speech therapists. Our rehabilitation model is based on a providing more therapy per day and more days per week to expedite your transition back home. It is a well-rounded rehabilitation program based in individual outcome-driven treatments delivered up to 6 days per week.

DO NOT SEND YOUR LOVED ONE OR ANYBODY FOR THAT MATTER TO THIS FACILITY! IT IS ABSOLUTELY HORRIFIC! PEOPLE WIND UP PASSING AWAY FROM THE NEGLECT IN HERE! DO NOT DO NOT DO NOT USE THIS FACILITY!!!! PS the only reason it has a star at all is b/c it makes you chose something. It really deserves a thousand star minuses!

I've been here many times and would never use the word impressed to describe my observations. Some of the previous reviews are accurate. When you visit this place you have to look listen and be very observant. Places like this should be visited between 10pm - 6am and especially on weekends and holidays. You should never I repeat never establish a routine, doing so will allow the staff to exploit the days and times of the day when they don't expect to see you. I visited a relative multiple times during the 4th of July week in 2018. One of the visits took place before 7am on a Saturday

They are located across the street from low rent appartments and a used car lot. There's always people hanging out in wheelchairs outside across the street.
The furniture looks like they found everything at garage sales- mismatched and run down. The beds are outdated with a crank mechanism to operate and cheap mattresses. The bed my family member had the first time didnt even work.
The whole place has an institutional feel. The food that was served when I visited was unidentifiable. I even tasted it and I still couldnt tell what it was.
I would not recommend this home to anyone.
