If you are preparing to begin your care at Mountain Valley Therapy, here is what to expect. Most patients come to us with a referral for physical therapy or occupational therapy that was issued by their physician - this referral is like a prescription similar in form to what you take to the drugstore for medicine and includes recommendations for treatment.
There are several ways to improve your experience with physical therapy, but it all begins prior to your initial evaluation. First, we encourage you to make a list of any questions you may have to ensure the time with your therapist is used most efficiently. Second, write down any symptoms you may be experiencing, and for how long.
The document below is to be used only by a Physician or other provider to prescribe a Plan of Care at Mountain Valley Therapy. To create a supportive environment in which employees can achieve their professional, personal, and spiritual goals and potential. Liars and very petty. I only opted to pay for my membership upfront and when I asked to pay another month I was told I didn't owe anything. I asked twice if they were sure and they said I didn't owe anything. A month later I get a bill in the mail. I made sure I was cancelled but apparently they messee that up as well and went to bill another month.
To make it fair I said I'd pay part of a month but since they lied to me by saying I didn't owe anything plus didn't cancel me correctly I shouldn't owe anything further. Several months went by without the manager calling and I get a letter saying
