Service to your loved ones is at the very heart of our mission to provide quality health care to our residents. Caring is our hallmark. Each resident is assessed by an Interdisciplinary Team generally consisting of Nursing, Therapy, Dietary, Social Services, and Activities Department. Together, with the resident, a personalized care plan is established.
Our Director of Admissions will be happy to guide you through the process to make it as easy as possible. You can be assured that we will do everything we can to make this simple for you. Below are some frequently asked questions and answers to move the process forward. DMA - 6, Level 1, History and Physical, current list of medications, and any medical notes.
We hire only those qualified applicants that meet our standards for excellence. At Parkway Health and Rehabilitation Center, staff members must have the required education, certifications, experience, training and personality to care for our residents or deliver the services they're employed to perform.

My mother was released today, the release process is terrible and long. After her release they gave me instructions on how to issue her meds that I would have to pick up at the pharmacy. Get to the pharmacy no meds have been sent by Parkway. Call Parkway and get the run around, left with no answers and my mom does not have her meds that she NEEDS every day and night. I HATE THIS PLACE

This is my first time dealing personally with a Rehabilitation and Nursing home but I am very pleased. The Admissions Director has been very gracious and concerned that I felt satisfied with the admission process and she displayed concern for my Mother which gave me a feeling of calm. She encouraged me to contact if I had any concerns or questions. I believe this facility will be a good fit for what my Mom needs to recover.
