Creek View is an excellent example of cleanliness, and care in helping patients regain their strength in times of need! Having experienced rehab in two other convalescence homes, Creek View excels far above the others. ALL of their staff, physical therapists ALL worked to ensure the best Care for the patient. Their dieticians prepare a wide array of meals to appeal to any pallet to pick & choose from, served hot. Thank you, Creek View!!!

My husband was a resident at Creekview Health Center for almost
one month. Over the last 7 years he has been at 2 other Skilled Nursing Facilities, Creekview by far surpassed the others! The therapy was outstanding and the nursing care was top notice. My husband suffers from severe pain and they were able to manage his pain within the first 24 hours, when the hospital was not able to. Dr. Kari was very responsive and easy to communicate with, any questions I had were answered within 24 hours. He missed his dogs terribly while he was at Creekview and Rosanne from activities made sure he was visited

Unfortunately, since my last review, my father (a resident/patient of Creekview) passed last October. My mother is still a resident/patient of Creekview. Dad did not have the Wuhan virus. He died due to believing that his family had abandoned him when Sacramento forced nursing facilities to terminate in person visits with family.
This is a tough review to write, as it's hard to give 5 stars when there was such tragedy in the past year, but we certainly can't blame Creekview. My dad was clearly impacted by the draconian and inhumane shutdown requirements imposed by Gov. Newsom and

More than ever, I believe that Creekview (CV) was the right place for my parents. The 5 stars I'm giving CV are well-earned. My 91 YO mother passed away in late 2020 and my 92 YO father passed away last month - both were on CV's Assisted Living (AL) side when they passed. Watching parents age/decline is never fun and finding the best care solution for them can be challenging. Mom and Dad's decision to move to Creekview AL to live out their days was one of the best decisions they've ever made. Everyone - from CV's Administrator Patrick, AL Director Vivian, and every member of Vivian's care team

My wonderful Mom took a fall, spent @ 7 days in the hospital, and the came time to transfer to a SNF (skilled nursing facility)... my father spent his last days at a local SNF, which will remain nameless at this point, and with that we felt STRONGLY that that location wasn't even under consideration. We did some research and hoped and prayed that she would be able to go to CreekView, even though she had never been here... her prayers were answered! Amazing! She will be discharged in a few days, after being here for almost a month. The entire staff is wonderful- too many names to list here that

I have waited a year to post this review due to extreme frustration with this facility. DO not let your family stay here. We experienced incompetent management and people who didn’t care. My parents were living here with no air conditioning last summer as they didn’t have enough money to fix it. My parents bought into a full contract with this facility and we were willing to lose the $40K to get them out. One example of lack of competence was changing the address. Since Creekview is a business address all residents who leave can’t do a simple address change. Management blamed us for not doing

My wife who is 81years old had major surgery and spent two weeks in the hospital before being transferred to CreekView Health Center for rehabilitation.
When we arrive I was beyond impressed by the facilities that had the appearance inside and out of a first class operation.
More importantly is the staff that provided the highest quality of care during her four week stay. The entire staff works as a team and it is very evident they truly care about every patient.

I'm very happy with the relationship Creekview and my family have built. The staff has been very helpful with needs of my loved one. My mother is the most important person to me. I'm sure my attention to detail, and my demands have at times stressed them. The challenges they havefaced during this time of COVID have not deterred them as they strive to keep their patients and staff safe. Tori and Lindsey are two of the best liaisons I have had the pleasure of dealing with. Big thumbs up you guys.

Nearly a year ago I moved from the Mendocino Building at Stoneridge to Creekview Assisted Living. I was coping with Parkinson's Disease and lost the ability to live independently. Creekview has an excellent well-trained staff particularly with top management who are very responsive to Resident needs and are pleasant to work with. Their first concerns are Resident safety and comfort. The meals suit me well. I like having linens changed weekly, the bed made, clothing laundered. But for me the most appreciated service and relief from stress is medication management including re-fills, doctor contacts

If considering Creekview SNF for a loved one, please ask about their Covid "observation". They roomed my mom, who was tested and negative with Covid symptoms at Stanford ValleyCare Med Center, with a patient who had a horrible cough and was ill. Several days later, my mom was in respiratory distress because she had gotten sick. While she struggled to breathe and kept tapping the nurse call light, she could hear the staff talking and laughing down the hall. I happened to call at the time, so I tried calling the nurse's station and nobody answered, several times. I was considering

My father was at Creekview for post-surgery recovery. When we visited, he continually commented how responsive and kind the nursing staff was and how clean the facility was. Therapy wanted to work with him more than he wanted though.
As a family, we were very concerned about dad's surgery, recovery and care. We were reassured and thankful that dad was so well cared for during his recovery. Admissions was very clear with information. The dietary group was responsive. Activities really understood dad's likes. I was kept informed by nursing staff when there were a few

My mother had severe, end stage COPD. She had been living in independent living at Stoneridge Community and was moved after a few hospitalizations to CreekView Health Center and eventually to Creekview Assisted living.
We found that though nothing is perfect, that the staff at both Assisted living and Skilled Nursing were kind and caring. They showed genuine personal care in my mother's complicated condition. It was not only the nurses and assistants / care givers, but the management, med techs, food service and engineering all showed concern.

I hoped I'd never have to place my 91 yr young mother in a rehab center. However it happend and when the hospital gave me three places to check out Creekview hands down was the best . She was very comfortable in her pvt room . The staff was outstanding !!! Millicent was my go to nurse she has great fallow up and we were on the same mind set . I'm a nurse as well so I know what to look for . Millicent stopped what she was doing at the desk to assist a pt who needed assist instead of calling for a CNA which would have made the pt have to wait longer . That speaks volumes when a nurse

My husband had an accident five weeks ago rendering him wheelchair bound while he healed from surgery on both legs. He went from a fully functioning lifestyle to requiring the services of a skilled nursing facility about which we knew zero! We had little time to react or research our options. We initially selected the location closest to home upon discharge from the hospital. After the first day my husband asked to be transferred elsewhere.
He was in his own room at CreekView within a few hours of the transfer request. I cannot overstate how happy we have been with CreekView and how grateful we

Hello, this is the worse place to have your family member. My family member was there last week when a resident died and they took the dead person through the the whole facility with the doors open so the residents could see. I asked the DON who name is Sarah who is always in her office with the door closed why the breakfast and lunch is always served cold, she directed me to speak to some one else. she is not pleasant does not know how to talk to residents or family at all. Why is she there the company needs to go in and see how the company is being ran. All they care about is a dollar sign.

My 84 year old mother went to Creek View after having hip replacement surgery. We found Creek View to be the best place possible for her to have her recovery. The staff was always kind, caring, friendly and professional. We were overwhelmed with the number of staff members that went "above and beyond" when caring for her. The outstanding physical therapy that she received was one of the key reasons why her recovery was so successful. My mom was more than just another patient at Creek View; she was cared for as one would care for a family member. My mom is home now recovering, and

I admit that CreekView was second on the list of skilled nursing facilities that my family was looking for to place my Dad in hospice care. Our first choice had no vacancy and so we came to CreekView for a tour to see what the facility looked like. Being fairly new, my expectations was that it would look nice. I am very happy that we went on a tour of the facilities. We were so impressed with the facility and the beginning process that my Mom decided right there that CreekView was the place for Dad and that we don't need to see another place. Let's face it - no one wants to put their parents into an Assisted Living environment. It's not an easy choice. But, sometimes, it's the right choice. Because we had to make that choice for both my mother and step-father, our family has been, as of this review, a part of the CreekView family for almost two years.
CreekView has an Assisted Living, a Skilled Nursing Facility, and a Memory Care Unit all under the same roof. In our time there, we have experienced all three sides and have been completely satisfied that this was the absolute right choice. During that experience

CreekView Health Center is 5 stars in all aspects and highly recommended. Having been to almost 100 such facilities, the worst and the best were encountered. CreekView is truly one of the best. There are excellent people that make this facility run so well. They are caring, compassionate and hard working. My 92 year old mother was in rehab and came out just about good as new. Thank you so much for the fine service.
