Services
We strive to create social programs and individualized activities to match the capabilities and needs of each resident. This also has a powerful impact on the healing process of those residents involved in some type of rehabilitation. A monthly calendar is posted to keep residents informed of upcoming events such as birthday parties, movies, musical programs, outings, candlelight dinners, ice cream socials, and other special events.
Your doctor has ordered the services you need for SNF care, which require the skills of professional personnel such as registered nurses, licensed practical nurses, physical therapists, occupational therapists, speech-language pathologists, etc. You require the skilled care on a daily basis and the services must be ones that can only be provided in a SNF on an inpatient basis.
We are committed to doing regular reviews and modifications of our sites in order to improve the accessibility for people with disabilities, and compatibility with assistive technology. We welcome your questions, comments and feedback to address any issues you may encounter and to increase accessibility to our website experiences.

Im not one to leave reviews but after my moms stay I needed to share so anyone who stays here can atleast have some advocacy over their family/friends care. My mother went in for roughly three weeks for pancreatic necrosis, she had a feeding tube and was there to initiate eating. They were informed by my moms GI specialist that she could not eat red meat or lactose, yet for all three weeks, even with reminders they still included those in her diet (leading to her throwing up for which they just gave her pills). The person sharing a room with my mom tested positive for covid and they neglected to

Stay away. From the beginning it is almost impossible to get anyone to answer the phones. On one occasion i was there watching through the glass door as my call was ringing at the nurses station as they stood around and ignored the ringing. The 1st night there my wife's cell phone charger disappeared So i brought her a new charger and of course her cell phone was gone. The day she was discharged i was informed they had her cell phone and they would put it in her purse. When she arrived she was missing her phone and 2 chargers. Also a inflatable mattress that was delivered by a medical supply company

The staff and management are handling it well in the midst of the pandemic. Everyone work together as a team. Rounds are done to ensure that PPE adherence are in place and skin and weights are being look into especially now that there is not too much activities and residents at risk for pressure injury and weight loss with Covid restrictions.
SSD are making sure that families are notified. Nursing are making sure that patients are being fed and hydrated. Staff are updated daily and management is transparent with cases.
Education are provided to fight covid, PPE use, etc.

No one answers the phone at any extension.
I have been trying to see my mother to check on her well being. She is supposed to be there getting care until she can be transferred to a skilled nursing facility. I would rate them as 0.
Not being able to get any information is appallingly and very wearisome. I guess they don't care until the insurance quits covering her then she will be transferred yet once again.

I’ve been at the SNF for four weeks now. The nursing staff has been very efficient, well-trained and caring. The activities department provided many interesting activities for me. The therapy helped to greatly improve my health situation. The social services staff was informative and very responsive. Overall a tremendously positive experience. I recommend this facility for anyone needing a skilled nursing facility in the area.

My elderly mother was at this facility for several weeks after a fall. They kept her drugged so heavily that she was mostly incoherent, then suffered severe bleeding and infections from lack of proper care. They also lie about terrible incidents that happen. My mother had to be hospitalized in ICU after 2 weeks of grossly incompetent care at this awful facility. While there are a few good folks there, the majority of help, the medical care, and the management is unbelievably unacceptable.

This location, while adequate as a care facility, seems to be quite well aware of the fact that they are one of the few of its kind in the area. Once your patient is admitted, your care will vary from person to person from poor to very good. From our first hand experience, it all seems to depend on the personal motivation of the employees and their point of view about their occupation: a calling or a paycheck.
However, the administration is absolutely horrid. Their performance is stereotypically government bureaucrat red tape style. Even when they are there they refuse to answer phones. During

I spent several hours visiting this facility on two occasions as part of my employment search. I met the staff, attended their meeting, visited or interviewed the administrator and his assistant. The team was very happy and there was a great upbeat vibe in their meeting. I found them to be of the high integrity and work ethic. Their goal is to do what is best for the patients and their families. The facility is modern, clean and nice in presentation. I have now toured about 10 facilities in a similar manner. There is a never ending demand on the staff at these facilities and often times the expectations
