Country Manor Senior Housing tenants choose from four distinctly designed communities. Communities differ based on a variety of elements, including lifestyle preferences, cost, characteristics and floor plans. Three uniquely designed neighborhoods offering 24-hour licensed nursing, daily activity programming, and supportive services like on-site rehabilitation and culinary, in a calming environment.

Country Manor's Long-Term Care facility maintains a 5-star CMS government rating for quality care. Specialties include progressive Memory Care and End of Life Care (Angel Arms). Providing inpatient, outpatient, sub-acute and in-home therapies. Country Manor's Rapid Recovery & Aquatic Center and Champion Rehab are separate entrance buildings, located on the Sartell campus.

The Foundation for Health Care Continuums is a nonprofit corporation described in Section 501(c)(3) of the Internal Revenue Code of 1986 and is exempt from federal income taxation. The management and direction of the business of the Foundation for Health Care Continuums is vested in its Board of Directors.

Since my dad moved in they have closed the bank and now he won’t be able to get his meds through the pharmacy because they don’t accept Medicare. Having these things available were reasons we chose that facility. While having lots of protocols in place for covid which made things difficult (although I appreciated this), he was in contact w/employees who were not vaccinated. For example. I couldn’t go in at one point when he had heart procedures, and the assistant that went in his place told him she wasn’t vaccinated.

That director in nursing, Wanda is something else👎🏽 Seems as if they are understaffed.. that's a BIG concern..

I had heard so many good things about Country Manor but when my mom went into the rehab section with a broken hip and Alzheimer's we were not happy at all. We cried a lot about the care she received, don't get me wrong there were good staff there too. The communication was horrible, within 5 weeks she had pressure sore on her heel, and bottom, which we weren't updated on. I as a nurse had to ask to look at her bottom and before looking they then decided to tell me about it. One day my sister was there and they got her up at 5:20 for dinner, didn't change her prior to getting her up. I was there

My sister and I were informed that my mom fell out of her wheelchair trying to get something out of the refrigerator. The lady who called said she might have broken her left hip during the fall. My mother already had the broken left hip when she moved in there. No repairments were able to be made to her hip. The staff there didnt know anything about her record of her broken hip which I find to be bull due to my mother just moving in there from the hospital with all the hospital records. I will defiantly be looking into legal actions.

My grandparents have been cared for here for about a year (assisted living and nursing home). The facility is spacious, well appointed--maybe a little bit lavish, but nice. The care provided has been satisfactory from my grandparents and family's perspective.

My Mother was Admitted from St Cloud Hospital, Room 531 rapid rehab, She had been previously at Talahi. I need not say more. I would like your staff, including Nicole RN supervisor and Social Worker to know how much we appreciate their professional yet caring and compassionate service. This is the best I've ever seen in this setting. I am a 35 year veteranof nursing and the staff at Country Manor are exceptional.
