Services
Co-payment and applicable Hawaii GE tax are collected at the beginning of each session in cash, check or credit card. For returned checks a $40 fee will be applied. For "no call/no show" or cancellations with less than 24 hours notice, you will be charged the full session fee as this is not covered by insurance.
