A large part of what makes a community special is the people. We want our residents to feel comfortable, respected and safe. Having a warm and friendly staff with clinical expertise is the key to making residents feel this way. Wayne County Nursing Home & Rehab Center has board-certified geriatricians on staff, including a medical director.
Wayne County Nursing Home (WCNH) is a 192-bed skilled nursing facility located in Lyons, NY. The facility is one of few in New York State to operate under a Household model. The old "hospital" model that became so prevalent in nursing facilities throughout the nation in the 1960s is gone. There are no large central nursing stations with residents parked around it in geri-chairs.
Our Bariatric services are designed to promote and maintain the resident's quality of life. Using a multi-disciplinary approach, dedicated professionals educated in the treatment of bariatric care work with the resident, their family, and their physicians to establish a plan of care. Our Bariatric Program provides skilled nursing or short-term rehabilitation for patients with body mass index (BMI) over 30.
Our activities department is here to meet your recreational needs while at Wayne County Rehab. Please join us in as many activities as you like. It is our pleasure to make your stay enjoyable both socially and emotionally. The department offers activities 7 days per week, including day and evening.

My mother came to the WCNH in January and I have been really impressed with the care she has received there. The nursing staff has been attentive and keeps me informed. I love that I can walk up to any staff in the hall and ask questions during my frequent, unscheduled visits and they have never brushed me off. Mom has dementia and even though she is not in the memory care unit, they have made accommodations for her. She is more engaged in life now thanks to the staff proactively and gently pulling her into activities even though her dementia has progressed. Mom even says the food is good! I’ve

This nursing home is beautiful, clean, and everyone is so friendly when we visit. It has a wonderful home like environment. Not institutionalized like most nursing homes. Staff go above and beyond to please their residents and their families.

I have never seen a place run so poorly that I am concerned for all the patients that go there. Administration does not respond, you can’t reach your family member to talk to them. They do not contact the family to keep them informed on the health of the patients. They order tests without reason. I had to call the ombudsman to help intervene with my family care. The worst was I requested a tranfer out of there and they did not honor my request.

Medicare FRAUD, staff withheld M.D prescribed medications, unqualified staff, poor infection control, nurses put patients at risk for falls, ring bell and wait 4 hours for pain medication... ok if you are not in need of skilled nursing care. Needs a drop in un ---announced investigation from state and federal authorities. Will report them.

This is one of worst places I have ever seen. The night staff is the worst. The staff is lazy and show no care. Patients are neglected. My mother devolped sores due to the non care of the staff. She was given unnecessary antibiotics and the medical staff is incompetent. Patient are left in their beds until someone complains. I observed residents that were hungry and forced to eat ketchup packets because they were not fed.

We placed my mother in law there in the Memory Care unit. All the aids and everybody we have meant have been extremely helpful and nice and called her by name and made her feel so welcome it was the best decision we made.
