To ensure our patient schedule is running as efficiently as possible, we ask you to arrive 10-15 minutes early to complete the necessary paperwork. New Patient Information - includes patient information, insurance, phone numbers, accident information, patient condition, health history, and medications/vitamins.
We are providers for Blue Cross Blue Shield, Medicare/Medicaid, Sanford plans, Avera plans, Dakota Care, Health Partners/Chirocare, Medica, Preferred One, and others. Please call and talk with our office manager to see if we are a provider for your insurance. Chiropractic care is covered under many insurance plans.
