Almost 35 years ago our founder Sheny Graham opened her first 2-bed assisted living facility in her own home in Long Island, New York. There she used her years of care giving experience to provide a loving home for elderly clients, as she simultaneously raised her own kids in this same home. A couple of years later, she was able to buy a facility in Jacksonville, Florida, and the rest is history.
Services
Her heart assisted living employs a variety of positions, including: nurses, caregivers, CNA's, med techs, housekeepers, administrative/clerical staff, cooks, and kitchen assistants. Fort Caroline Gardens is a 78 bed facility located in a nice and quiet residential neighborhood on Fort Caroline Road. Although we have many residents who are healthy, alert, and independent, our staff experience allows us to provide a home where clients can age in place for many years to come.
We have a team of staff experienced and trained in providing a high level of assistance with all activities of daily living and also caring for those with dementia and Alzheimer's.We have nurses on site seven days a week. We have a variety of room styles, including both private/semi private rooms, some with kitchenettes and private bathrooms.

Autumn Village is an 80 bed facility located in the heart of historic riverside, three blocks away from St Vincent's hospital. We have a beautiful secure and large courtyard with beautiful landscaping. Many of our clients enjoy the flexibility and freedom of being able to walk in the riverside area, visiting the shops and cafe's in riverside.

Autumn Village helped us tremendously. The owner Danny is a great person to work with and worked with us to coordinate care for our sister. The staff is exceptional and treat the residents as if they are family. The facility is located in Riverside and is a very peaceful setting with a fountain and courtyard. They maintain the inside and outside to the highest cleanliness standards. I would not hesitate to recommend Autumn Village for anyone in need of quality care.

My grandmother stayed here for 6 years (2014-2020) and when the staff finally got comfortable enough with her dementia/inability to speak up for herself, they began stealing all of her nice clothes and shoes, even socks and underwear. We threatened Sheny Graham (mother of Danny Cano) with legal action and then suddenly, all of my grandmother's belongings, along with other people's belongings, magically appeared in a pile on top of her bed.

Autumn Village has been an amazing place for my family. Staff is very loving and provide exceptional one on one care for every resident. Everyone is met with a smile and a hug. Facility is very home like and very clean. Mandy is an awesome manager who assists staff whenever needed. Mrs. Sheny and Danny have put their heart into making Autumn Village a true home for their residents.

My mom was there almost 3 years it took her a couple weeks to get adjusted and realize she wasn’t going back home but then the staff were friendly with her and she was happy there and till she got to where she needed more than assistant I highly recommend Fort Caroline Gardens and all the stuff out there I’ve never smelled any foul odors and was always clean

Warm and welcoming. The staff all introduced themselves to my and my grandmother. Everyone came up to my grandmother throughout the day to say who they were and how nice it was to meet her. They truly made… her and myself feel welcome. Even the kitchen staff spoke with us and asked about her food preferences. When she goes for radiation, they prepare her food the way she needs to be eating for that visit. Everything is very professional, but with a touch of that at home feel. I absolutely love this facility. They truly are a great group of people.

Unfortunately due to an initial lack of understanding, we did not have a good experience. Excellent day to day staff, very informative but our specific problem was owner, Danny. ALL is behind us now. In all fairness to Danny, he was given second hand information and while he does his best to work with the resident and their family (us) ; hopefully; you don't or won't have a loved one who is improperly medicated. Once we took it upon ourselves to work with a new group of physicians, our loved one is doing incredibly better. The facility, Autumn Village, is clean, the rooms for residents very well

Her Heart (Autumn Village) in Jax, Fl is named appropriately. Sheny and her family truly love the residents and go out of their way to help them feel at home. I know because my twin lives there. And my twin sister is my heart:) She feels at home at AV and loved. Thank you for truly caring for people especially those in need.

Would NOT reccommend this facility unless client has no thoughts or feelings of his own.
If you are a sheep, have no opinion of your own, willing to eat ANYTHING put in front of you, and basically don't care about your rights, then Autumn Village is the place for you.
You can even let management take over your retirement check(s). Leaving you to be classified as intellectually disabled with relatively NO income to spend on yourself.
I CANNOT & WILL NOT endorse this facility. Been there, so thankful I am out!!!!!

My brother suffers with bipolar disorder and some of his ups and downs can get severe. Fortunately, he is 100% non violent and is mostly a sweet guy. He likes the staff and they like him. They've shown endless patience with him and he tests everyone's patience. Residents here follow a strict hands off of each other policy.
When I take him out for lunch or shopping, he always says, after a few hours out, that "I want to go home to Autumn Village."
He can't be managed in the family environment, but the staff at Autumn Village manages him and cares for him and that must be why he feels that they

Excellent facility. The owner & her staff care for the clients like personal family members. Clean, up to date facility. Staff are held accountable and no abusive or inappropriate behavior will be tolerated. The grounds are wel cared for, the clients are treated to outings and other things. The meals are hot and fresh.
