Please complete the New Patient Intake Information Forms (Including your insurance information) and the Patient History form and bring them with you to your first appointment. I cant thank Ken and his awesome team for getting me healthy and back to work . A very professional team . There only interest is you! I had 2 major surgeries in 3 mos. Both treated by Columbia Physical Therapy . I'm back to work earlier than the surgeons thought . Dont go anywhere else for therapy.

Great team at Columbia Physical Therapy in Othello, WA! Together they’re helping me get back to my healthy body. My knee injury was causing some issues that would conflict with my work; I was in daily pain and discomfort. Luckily with PT my pain is gone and my knee is back to normal. I’m currently seeing Ken for my neck and already I’m seeing some improvement with only 2 visit in. I’m confident he and his team will get my neck to a healthy state as well! Thank you CPT! You guys are amazing!
