As a non-profit homecare provider, SummitWest Care believes that people deserve to heal at home. Our experienced nurses and homecare specialists provide services to both private insurance patients as well as Medicaid, Medicare and other underserved individuals. We are committed to filling the gaps in the homecare community and providing services where the demand is heavy in the Grand Valley.

We are proud to provide both medical and non-medical homecare services to all ages of our community. SummitWest Care is based in Grand Junction, CO and proudly serves Mesa, Gunnison, Pitkin, Eagle and Garfield counties. We serve those who are ill, temporarily disabled and those who require long-term care.
Services
In 2002, our community recognized the need for providing quality care in the home. As a result, SummitWest Care was developed as a non-profit, free-standing homecare provider. SummitWest Care was designed to serve people in need of homecare who did not know how to get the help they deserved.

Since our inception, SummitWest Care has served thousands of clients and families in Mesa County and have had the support and donations from organizations such as St. Mary's Hospital, Rocky Mountain Health Plans and Hilltop.Because of our strong staff expertise and leadership and the profound belief that people deserve to heal at home, we have received several awards for quality and excellence in home healthcare service.
SummitWest Care's highly trained, compassionate team delivers a full-spectrum of healthcare needs, offering both medical nursing services and non-medical homecare services to the Grand Valley and beyond. Our team is experienced with patients of all ages, whether you need chronic illness management, pediatric homecare, or house keeping and general assistance, we promise all our clients the best possible care and support.
Having an established primary care doctor is vital to receive home health care. Doctors follow care, approve orders and work side by side with us to ensure you're getting the best possible care. To be homebound means you have trouble leaving your home without help because of an illness or injury, or leaving your home isn't recommended because of your condition.
There are several benefits of telehealth and remote patient monitoring for both healthcare providers and patients. Benefits range from cost savings, avoided ED visits, patient satisfaction, stress reduction, peace of mind and beyond. SummitWest care is proudly offering telehealth services to improve the quality of care and continuously monitor patients from the comfort of their own home.
SummitWest Care is the only agency on the Western Slope of Colorado that provides specialized Newborn and pediatric in-home care. Caring for a newborn can be overwhelming, especially when your newborn is premature or high risk. We are proud of our highly-skilled program that offers support to families with newborns and children who have special needs.

I have grown up in Grand Junction my whole life. All of my family lives here including my grandparents. My grandma recently took a hard fall and started loosing her memory. SummitWest Care was there to help right away. They helped her get right back on her feet in her own home. Not only did they help her physically they also helped her emotionally. They were kind and there for her. They met all her needs and made things easier for her so she could move around. Not only did they treat her well they also listened to all of us as a family and met up to our expectations. SummitWest Care is an amazing

If this review was focused on the nurses, it would have 5 stars. But the office staff has proved themselves to be less than stellar. I won't get into personalities, but if you look up "passive-aggressive" you'd find some of their photos. I was a client for almost a year. The nurses, especially Suzie, gave me excellent care. Having a wound vac was scary, and they made that all better. I will miss them.

Since I live so far away, having reliable people to look after my mother is essential. Everything about the way these people go through the routines that are part of taking care of an elderly person is caring and first class. The reports I get from Mom about her impression of the care she receives is very reassuring. Believe me, if she were not happy, she would let me know!
