Services
Maple Ridge Care Center is a 75-bed, Medicare-certified skilled nursing home and rehabilitation center with private and semi-private rooms. Our staff includes licensed nurses and physicians, CNAs, and extensive support staff led by our administrator. We are a 501(c)(3) non-profit community and rely partially on tax-deductible donations to continue serving our seniors.
The mission of Wisconsin Illinois Senior Housing Inc. (WISH) is to serve elderly and disabled individuals by providing quality housing, healthcare and other services to enhance the lives of those we serve. We're a nonprofit 501c3 senior living community which means we are mission-driven, not profit-driven.
A short term rehab stay is sometimes required after a hospital stay to continue recovery from surgery, illness, injury, stroke, or other conditions. Our expert therapists work with you to continue your recovery, help reach your health goals, and rebuild and strengthen to get you back to your independent lifestyle.
Transitional care coordinates patient care when someone receiving care physically moves from a hospital to a rehab center or back to home. A move usually means a change in your care and the people who provide it to you, so it's important that everyone communicates effectively from one location to the next so you receive the care you need.
Taking care of a family member can be rewarding and emotionally and physically draining, often leaving little time for other parts of life. Consider us when you need a break. Respite care takes over caregiving duties for temporary periods of time, giving you a break and providing peace of mind knowing your loved one is receiving the same excellent care you would give.

My dad was there for rehab after breaking his femur and I don’t have one good thing to say about this place. One time when he was done in the bathroom and pulled the string, it took them over an hour to come and help him. He’s timed them since, and it’s NEVER been less than 30 mins for anyone to come. We picked him up today and he said he set his stop watch this morning after pushing the nurses button and it was OVER 2 hours before anyone came! One day I visited and his garbage can had some white splatter all over it that I simply wiped off with a wet towel. My mom said there was huge spill

I am a happy resident at Maple Ridge. No facility can possibly be all things to all people. In my opinion Maple Ridge does an outstanding job. I have nothing but praise for the staff. They definitely do the best with the resources they have. Remember, an attitude of gratitude goes a long way in soothing the soul. Thank you for being here Maple Ridge!!!

I have been the Rehab Director at Maple Ridge Care Center for more than 10 years now. I love coming to work every day to see the residents and their families. There is a strong team of hard-working people in the facility, and I am happy to be a part of the Maple Ridge team. After working in other long-term care facilities, I can say that Maple Ridge is a very nice, clean facility- the outdoor space is beautiful as well. Overall, a great place to spend time. :)

My mom has been at Maple Ridge Care Center since December 2009 & my father recently joined mom in February of 2020. Maple Ridge has done an outstanding job caring for my parents. Prior to the lockdown, we often had family picnics on Sunday. We would stay for hours visiting with our family and other residents. Maple Ridge was an extension of our home because mom was there. Since dad moved in the lockdown has been challenging but staff has gone above and beyond to make my dad feel comfortable.
