Our skilled staff can quickly and easily get you set up with the in-home services you need. Canyon Home Care & Hospice is a provider for hundreds of insurance panels. Our Benefits Coordinator will assist you in determining what your policy will and will not cover.
With a physician's order, we routinely coordinate pharmacy needs, DME, O2, Infusion, Enteral feedings, Diabetic and Wound care supplies etc. Advanced notice of a discharge is always helpful and appreciated.

Canyon Home Care & Hospice is a provider for hundreds of insurance panels. To learn if we can accept your insurance for coverage of services, please call us at 801.485.6166 and ask for our Benefits Coordinator. She will assist you in determining what your policy will and will not cover. If you need help, and we cannot serve you due to insurance restrictions, we will be happy to assist you in finding a reputable substitute.

Phenomenal group of clinicians who always strive to do what’s best for the patients. Canyon Colorado has a small town company feel where decisions are made based on what’s best for each location. The office staff are friendly and easy to work with and the clinicians will go above & beyond for the patients they serve.

I’ve been receiving infusions for a few weeks now and have been blown away by my care team. Sabrina, my weekday nurse, did it all from coordinating care with my doc and my pharmacy, training my husband and making return visits when my line blew. Joe, my weekend nurse, with compassion and expertise made the recommendation for a picc line which made all the difference. I’m so grateful to the whole team!

Our experience with Canyon was with both Home Care and then eventually with Hospice. Our entire extended family interacted with the team who took care of my mother when she was sick and had hope in her treatments, and, keeping her comfortable and pain free with dignity and grace when she was transitioning on. We simply cannot praise the Canon team enough as our mother Mary got the highest caliber of care and was in excellent hands. Sabrina is awesome. The care, skill and professionalism she provided was everything we could have hoped for. Prompt in her arrival. Skillful in her care. Patient with answering all of our questions, including training for after she left. She stayed in contact with us during and after hours. Going above and beyond, she helped coordinate the delivery of additional supplies. We are grateful for her level of care and lucky that we where able to work with her.
