Monticello Nursing & Rehabilitation is best known for our Rehab to Home Program, which is a short-term skilled rehabilitation program where more than 83% of our residents return home! During your short-term rehabilitation stay you will have access to the best physical, occupational, and speech therapists.
Whether getting your strength back following an illness, recovering from surgery, or recuperating from an injury, our outpatient services can provide just what is needed to get you back to enjoying life. We work with all age groups so if you are prescribed therapy and want to stay close to home, ask your physician about completing outpatient therapy at Monticello Nursing and Rehabilitation Center.
Transitioning to Long Term Care is a difficult decision for everyone involved. Monticello Nursing and Rehabilitation walks families through this process, with the resident's quality of life and care as the top priorities. Long Term Care not only provides a full array of nursing and therapy services but also life enrichment activities and convenience services.
