This was made capable by the University of Alabama, Capstone College of Nursing, and Professor Dr. Jeri Dunkin; the initial funding was received from the Division of Nursing, Bureau of Health Professions, Health Resources and Services. Capstone Rural Health Center became a Federally Qualified Health Center under Section 330 of the Congressional Public Health Service Act in 2007.
Capstone serves Walker County, Alabama and surrounding area populations by providing comprehensive primary care services that treat illness, as well as promote & maintain health. Capstone incorporates prevention and health promotion, treatment of illness, and management of chronic diseases - often all during the same visit.

We have Certified APplication COunselors (CAC) available at all Capstone locations to assist with your insurance enrollment needs. Capstone Rural Health Center has Certified Application Counselors (CAC) available and ready to assist with any of your insurance enrollment needs and questions.

Capstone Rural Health Center is always accepting new patients with Commercial & Private Insurances, Medicare, Medicaid and Uninsured for Telehealth visits.
Capstone Rural Health Center is always accepting new patients with Commercial & Private Insurances, Medicare, Medicaid and Uninsured. Capstone provides all levels of primary & preventative health care services. Patients should arrive 30 minutes early to fill out all paperwork. Copayments or Nominal Fee are due at time of service.
As a Federally Qualified Health Center, Capstone Rural Health Center provides a Sliding Fee Discount Program for all patients that are uninsured or underinsured and whose total household gross income falls at our below 200% of the federal poverty guidlines. The sliding fee discount is used to calculate the basic discount and is updated each year using the federal povery guidelines.

Receptionist and all the staff are so nice and helpful. It is a great place to go to a Doctor for $20.00. They have their own pharmacy on site and that is very convenient. If they can't help you they will help you find the right place to go. It was very clean and welcoming. We are blessed to have Capstone Rural Heath in our little town. ⭐⭐⭐⭐⭐.

Wish there was the option to do negative five stars. I called and scheduled an appointment as one is expected to do. I take time off work shift my schedule around and 30 mins before my appointment this morning while I'm driving to the office I get a ride phone call stating that I will have to reschedule because the physician I was to see just didn't show up for work. I explained that I really need to be seen for some issues I am having and that I would gladly go to another location. She was rude and snappy and said I would have to reschedule without even taking time to look to see if I could be

This place has caused me nothing but trouble.. I see Dr Miller see doesn't give me enough prescription refills she gives me one and I see her every 3 months I have to call every month to get refills and when I go see her she acts like she's took a handful of xanaxes she can't keep her eyes open I'm trying to find another doctor as I speak so we'll see

Dr. Haldar is the best. He really cares about you. He always takes the time to listen to his patients. All of the staff is always so sweet. Plus you don't have to wait long at all before they call you and in a room before maybe 5 minutes Dr. Haldar will come in. Great place. Plus now they have a dentist and eye center. And the pharmacy is right inside.

Great place it helps so many people who don't have insurance and they now have a low-cost dental clinic. I have insurance but I know a lot of people that don't in this place is a lifesaver for them their medications are even reduced in price so most people with any income especially those with very low income can't get there life saving medication without spending every time they have

They work towards making sure you're healthy and even have options for uninsured people to see a doctor. Lord forbid your insurance run out though. The front desk staff doesn't relay messages to your doctor and you end up going without blood pressure medicine for a month. Then when you finally get to see the doctor, they're pretty upset that their receptionists didn't let them know that you're going through sort of a health emergency.
