Services
Pleasant View Skilled Nursing & Rehab is a 96 bed skilled nursing facility located in the rolling hills of southern Green County, allowing residents a picturesque view of the rural landscape. We've provided 24-hour rehabilitative, long term, and dementia care since 1969. Pleasant View has a rich history that extends back to the 1860's; the first poor farm for Green County was purchased in 1861 in the town of Mount Pleasant.
When I first was approached about joining the Friends Board, I was somewhat hesitant, due to the heavy involvement I have with other organizations. But having volunteered at our great facility for quite a few years, assisting with Club 48 meals, early breakfasts, Patio picnics, and other events, I realized this was just an extension of what I currently do, but on a new level.
Recognizing that your loved one is no longer able, or safe, to be at home is the first step. We want you to know how hard this decision is, but we are here to help during this transition. These meetings are a good time to ask questions about your loved one's stay. The Nurse Care Coordinator, Social Worker, Dietitian, and a Life Enrichement staff member typically offer input.
OUR MISSION: To strengthen our communities to care for our own and keep our loved ones close to home. The O'Rourke Dementia Stabilization Unit (DSU) is the result of a consortium from the four counties of Grant, Green, Iowa & Lafayette.
Concerned members of the community recognized the need to care for our older adults with dementia and envisioned a compassionate safety net for those suffering from distressing and sometimes violent symptoms of dementia.This brand new unit and the only one of its kind in the State of Wisconsin offers: Short-Term Respite, Medical Evaluation, Psychiatric Assessment, Social Evaluation, Education for Caregivers & Behavior Support Plan.

The doctor may make a recommendation for stay at a skilled nursing facility to allow for recovery and/or rehabilitation. The Family or Resident Representative is invited to call to schedule an appointment to learn more about the nursing home, to tour the nursing home and begin the application process ask for our Admissions Coordinator.
