Grace ADHC Center is a licensed day health care program by California Department of Public Health that provides a combination of medical, social and therapy services to adults who have difficulty functioning in their own homes. Our highly qualified staff can provide a structured day program which includes nursing, meals, transportation, social services and restorative therapies such as physical, occupational and speech therapies, in a warm, caring and secure environment.

Grace Adult Day Health Care Center (ADHC) is an organization that provides our elderly with full range of health care, rehabilitative, social services, balanced meals and transportation. Grace ADHC is an approved Community Based Adult Services (CBAS) provider. Participants spend quality time with people of similar age and background and participate in therapeutic recreational and social activities that encourage healthy, independent lifestyle.
A high chance of further deterioration and probable institutionalization if ADHC services were not available. Enroll in a Managed Health Plan by calling 1-800-430-4263. If you live in Santa Clara County, you can chose between Anthem Blue Cross and Santa Clara Family Health Plan. If you live in Freemont area (Alameda County), you can chose between Anthem Blue Cross and Alameda Alliance.

My experience with Grace adult day health care center has been the best caring, loving and nurturing day care center of all that I researched. my mother is very difficult and they were able to handle all of her issues and send her home with a smile everyday which in turn created a big smile on my face. I'm impressed with how well thier staff take care of Alzheimer's, dementia, and other sensitive health conditions with such ease which gave me peace of mind.. I highly recommend them to any who are ready to stop looking and find a good home for their aging loved ones.

I love this place, i take my mother here and i would recommend it to anyone. The staff is friendly and inviting, the manager is amazing and very helpful. I love how nice and great they are to the elderly, its just been amazing to see my mother have so much fun and love this place. I am thankful to everyone in here. They have been great to me and my family which means the world to me.

Shortly after my mother had a stroke in 2014, research into the options for quality elderly assistance in the Bay Area and revealed that Grace Center was the best option.
