When curative treatment is no longer an option, or when the decision is made to pursue comfort versus cure, hospice becomes the treatment of choice. We don't always see the subtle changes of decline. The discussion of beginning hospice care may start with a health professional, but it doesn't have to.
Hospice is comfort care for those facing life-limiting illness. The goal of hospice is to manage symptoms and provide comfort while helping to live each day with quality and dignity. Care is provided by a team of hospice professionals: Physician, Nurse, Social Worker, Chaplain, Health Aide and Volunteers.
Most individuals and family members know when end-of-life is near and desire the best in care for this phase of life. We couldn't be happier with the care and assistance by the entire team at Horizon. Everyone was genuinely concerned to keep her as comfortable and well cared for as possible. One of your caregivers assured me that she "pampers" all of her patients, and watching her and hearing of how she was with my mom, she certainly does. THANK YOU ANGIE!!! If hospice service is ever needed by one of our family members in the future, it will certainly be Horizon. Thank you all again for such amazing care to my mother.

Firstly, they train you for a week as if you’re getting certified for a skill and your not so the week training is honestly redundant. They expect and require too much, they want you to change your whole appearance just to work for 11$ an hour. Their supervisors are very rude and prejudice, I had a doctors appointment and they told me I couldn’t go to it or else I would have to do another weeks training all over again. They aren’t lenient at all and they don’t genuinely care about you or don’t try to work with you. Horrible company I would never apply for or work here again I’ve never

Wonderful experience!!! Highly recommend Horizon Home Care! My 93 year old father was prescribed home care after a 3 day stay in the hospital with pneumonia. A RN, PT & OT came to his house 2-3 times a week for a month after being discharged from the hospital. All three were wonderful individuals! They all were very patient and kind with my dad. And they all took the time to answer my questions and go out of their way to look for answers. They followed through in a timely manner. Both my dad and I felt very comfortable with this team. Have had nothing but good things to say about them. Megan is my Case Mgr RN... she is always consistently great, offers great advice on certain health subjects, and really has helped me through some really rocky health times. She has always been there to bat when supplies were missing, and goes the extra mile.
All in all... through my TPN feeding to now needing fluids regularly due to chemo, she has helped me navigate the health side with ease and has helped take that stress off me. I can't thank her and Horizon enough!

Garden View at Community Memorial Hospital, Menomonee Falls. Our physician set-up the arrangements for my father. Everything is first class; from the staff to the rooms themselves. They read to him, sing to him, shave and bathe him daily despite the fact he has not been conscious since day one. In my absence, they keep his favorite songs playing on his CD/tape player 24/7 - I don't have to ask. Sorry to say, if the staff at his assisted living facility and the rehab facility was a fraction as caring as those at Garden View, he probably would not have end-up there. BTW, I very seldom write reviews

Horizon Home Care & Hospice was there to help me when a family member was in need. Everyone I worked with at Horizon was fantastic, but Laura H., the occupational therapist assigned to our case, was exceptional. She treated us like family, and the kindness and support she showed me, the caregiver, will never be forgotten. She truly cares about her patients and their families, and it comes through in all she does. Thank you Laura, and thank you Horizon!

However, administratively the company is horribly run.
My mother was cared for by Horizon during the last 2 1/2 months of her life. In home care was for the most part, handled well with qualified staff.
Upon receipt of the billings, it became necessary for me to review each and every date of service, cross check the hours of staff and the billing amounts. We were over-charged hundreds of dollars and if had not been for my business expertise, we would have paid considerably more than what we owed.
