Services
In 1916, Community Nurse Home Care started with one woman who traveled by bicycle to neighbors' homes to care for them when they needed it. Over 100 years later, Community Nurse Home Care (CNHC) has grown to a team of more than 250 nurses, therapists, home health aides, social workers, administrative staff as well as a large group of dedicated volunteers.
Community Nurse Home Care provides personalized home care services with compassion and respect for every patient. Our comprehensive services are designed to optimize your health, empowering you to remain at home with the maximum level of independence. If you or your loved one plans to be - or is unexpectedly - admitted to the hospital or a skilled nursing or rehab facility, call us.
Your support is critical to providing innovative, lifesaving services to our community. The generosity of our donors allows us to provide visits when we feel they are necessary, even when they aren't covered by insurance. Some of our most critical, unique programs like Memory loss Support Consultations, Tele-monitoring, Care Transition Calls and Community Health Education all rely on your generous support.
With the help of volunteer committees and generous sponsors, Community Nurse Home Care produces annual fundraising events to support our patients' health and quality of life. We host a variety of events from our very popular Bingo Night to educational events to more formal evening fundraisers.
