Community Homecare

Community Homecare is a locally owned family business. We have provided home health services and consultation in East Central Illinois since 2004. Our Administrator, Kelly Scott and Director of Nursing, Kim Coy are nurses with over 30 years of home health experience. We are dedicated to offering residents of East Central Illinois the highest standards of home health services.

Our registered nurses are licensed and trained to help you on your path to improved health and safety. They work closely with you, your family, and rest of your care team-at the direction of your physician-to assist in your recovery.

Nursing services include: Completing a full assessment and creating an individualized care plan Administering needed treatment Working with your pharmacy on any medication changes Working closely with your physician and documenting any health changes Coordinating services with other caregivers (e.g., Physical therapist, home health aide) Cleaning, observing, and dressing any wounds Teaching basic tasks of care to family members Our RNs are specially trained and able to bring the best care to you.

Many people don't realize that anyone can call our agency to make a referral. Community Homecare accepts referrals from hospitals, friends, family, social workers, physicians, or yourself. If your doctor decides you need home health care, you have every right to choose a home health agency to give you the care and services you need.
