Compass Home Health & Rehab, LLC was established in September of 2015. It purchased an existing agency (Personal Care Home Health Services, Inc) in September of 2016 that was established in 1986. We are a State Licensed, Medicare-Certified, and Accredited by the Joint Commission Home Health Care Agency.

Services
The following are professional health services that are delivered to the sick and disabled in their home, by professional health care personnel. Any person confined to their home due to illness or disability who require intermittent skilled nursing care. Detailed information regarding our services can be obtained by calls made directly by a physician, relative, friend, hospital, or social agency to our office.
Many skilled nurses and therapists have been taught, either through their academic teachings or employer, when patients reach their "max potential" or are no longer showing "improvement" with their care, they are to be discharged from skilled services.
Albeit this is how the majority of our healthcare model continues to manage its patients, our dedicated home health team is competent with managing care for those patients who require on-going therapy or nursing to maintain their chronic, long term conditions.What is it?
