Crawford Health and Rehabilitation Center strives to provide patients with a home-like environment while keeping them safe and promoting their well-being. We provide a full range of 24-hour nursing services with physician oversight. These include a restorative nursing program, therapeutic whirlpool, a spa program and wound and skin care.

When I came here I could not walk, I could not use my legs, I did not know my own name; I was just lost.
The need for short-term rehabilitation can happen instantly. Second time we have used Crawford Co Rehab to prepare my Dad to return home from a hospital stay. First time in 2018, was a good experience but in May of 2020 during COVID it was not good. He transferred from Baptist Hospital in Fort Smith after recovering from a blood infection straight to Crawford Rehab for what was to be his 21 days to gain his mobility back to return home. Suddenly after 10 days we were told they were releasing him with no explanation. Remember COVID restrictions had not allowed us to see him during his hospital and rehab stay. I’ve worked at this facility for 10 years now. It has always been a joy and a pleasure being part of the care of the residents at Crawford Healthcare and Rehab. When it came to the dreaded time that my grandmother was no longer able to live alone and needed 24 hour care, I knew that she would get the best care at Crawford Healthcare and she did! She made so many new friends in her short time there. She was loved. It’s very unfortunate that I had to even give 1 star to this facility because the care administered to their residents is sadly non-existent. My mother has only needed care for maybe a 2 wk period, and this nursing home has not only NOT assisted, they have been grossly negligent by not doing simple things such as bathing, hygiene, treat, and chart incidents that have come up while my mother has been under their care. After speaking with staff as well as the DON regarding a serious issue concerning my mother, I was told they would “handle it tomorrow”. I’m absolutely disgusted that an entire

This facility has done a complete 360 ,I was visiting today and they have lots of specialized trained very friendly staff.The halls smelled really good and I noticed the staff were very attentive to the needs of the residents. I was very impressed with the cleanliness of the facility as well as the highly trained attentive staff. I have been a certified nursing assistant assistant for 26 years and I would recommend this facility to family.

My grandmother was NOT treated well at this facilty. Her Exelon patches were not removed daily when the new one was applied. When she arrived at hospital she had three still stuck on her back. Negligence is an understatement. Don't put your loved ones here if you care about them. This is not a Health Care facility, rather a dumping ground for those who they are just waiting to pass.
