Kosciusko Home Care and Hospice is a not-for-profit, independent corporation, Medicare and Medicaid certified, Indiana licensed Home Care and Hospice agency. We are located in beautiful Kosciusko County, home of over 90 lakes, rural farming communities and the "Orthopedic Capital of the World." Our offices are in the K21 Health Services Pavilion in Warsaw.
Our agency was founded in 1976 by eight local nurses who saw a need for home care services in our community. In 1982, we added hospice services to serve the needs of the terminally ill. Our mission is to provide quality health care promoting the highest level of independence and well being to any who utilize our services.

Services
Sarah has been with Kosciusko Home Care & Hospice for nearly a year and serves as our Intake Coordinator. Sarah has been in the medical field for six years, but her roles were always with direct patient care. She was attracted to this role because it was going to be a new challenge and help her further her knowledge and experience.
As we continue to strive to provide exceptional Hometown Care, we ask those of you who have experienced our care, firsthand, to let us know how we did! The future of health care is a serious concern for all of us. There are many unknowns, but our dedicated staff knows one thing for certain - we want our agency to be available in Kosciusko County for the next family who needs our help.
Hospice staff members work together as a team to support the patient as well as the patient's loved ones. Hospice care is not limited to a person's home. Hospice care can be received in a variety of institutional settings including: nursing homes, assisted livings, group homes, and the hospital. The healthcare team sees a person's place of residence as their home and gives all patients the same care and services regardless of location.
People respond to the dying process in so many different ways but there are signs and symptoms that seem to be typical of the general population. The information presented below is merely a guide and an informational tool as to the types of changes you might see in yourself or your loved one. This is marked by a decreased appetite, desire for food, decreased amount of food eaten.
Comfort care is the main focus of the hospice philosophy. There are times during the disease progression that symptoms may occur that compromise the patient's comfort. Hospice uses medications and many other techniques to attempt to control these symptoms. Pain comes in many different forms and encompasses many different facets of an individual.

We attempted to use this organization for both my mother and father. They are horrible! I would not wish them on my worst enemy. With my mother they would not listen to our concerns, she got much worse under their care and the doctor gave diagnoses (that were proven wrong) without any testing and while having severe anxiety told her it was ok she was going to die soon anyway. We took her out and she is doing the best I have seen her in years! My father got very little comfort from them and after making a trip for the ER regarding something not related to Hospice was removed and our request to reinstate

This agency went above and beyond to see that my father was well cared for before his passing. All of the staff were compassionate and very knowledgeable. My family and I couldn’t have asked for a better support team. I have personally seen them care for others in our community with the same compassion and respect as they showed my loved one. They truly have a heart for what they do.

Kosciusko Home Care & Hospice is the largest tenant in the Health Services Pavilion. They are a great group to work with. While we don't necessarily use their services, we do encounter clients from other tenants who are very complimentary of their work. It is a joy to have them part of our Health Services Pavilion family!
