Our mission is to offer quality health and housing services that promote independence and well-being for those we serve. Stephanie has over eleven years of experience in the healthcare industry and has spent this time predominately working with senior living communities. Additionally, she has experience in a variety of different sales, marketing, and business development roles.
Elmore Assisted Living & Memory Care, founded in 2015, is an exciting addition to the town of Elmore, MN. Our focus is to provide modest, economical housing accommodations that promote independence and well-being, with support available when needed. Residents live simply while not sacrificing quality of life.
Our Elmore Assisted Living & Memory Care Community is separated into six separate neighborhoods on three floors. Each floor is comprised of a west wing which houses our male residences and an east wing which houses our female residences. The first and second floors house our traditional assisted living residences, while the third floor houses our memory care area.
Along with monthly rent, each resident will also be assigned a customized level of care - from Enhanced Assisted Living Care to Ultimate Memory Care. Because we're focused on serving high-needs assisted living and memory care residents, these care levels provide the value families are looking for paired with the care that residents need.
We founded Sunflower Communities with one goal in mind: to create a place that encourages closer connections, fosters stronger relationships, and promotes a more vibrant way of life for each of our residents. Shared Spaces Promote Safer Living - Our communities are designed for resident safety, increased interpersonal interaction, and daily engagement.

I worked their for about almost a year. I worked in the kitchen. it's drama. one of the kitchen people smoke pot the dietary manager doesnt do anything to stop the smoking pot. I quit cause i was getting verbal and emotionally abused by one of the workers..yes, she still works there I wouldnt want any of my senior friends or family in nagels care. she talked to me like third person.i called corporate they didnt seem to care or what to take things into matter. I even told the head man who owns that place he didn't care.

To date we have several clients living at both Elmore and Nagel Assisted Living & Memory Care, all of them are well taken care of and thriving. Had it not been for their open mindedness and willingness to get to know our clients these individuals would have most likely been prematurely placed in nursing homes. Thank-you Stephanie for coming in to our case management offices to educate us on the excellent housing services you offer and continuing to update us on availability- we all appreciate it!

I have to say how impressed I have been with Stephanie Bauman from Sunflower Communities. My job as a Relocation Coordinator finds me spending countless hours on the phone looking to move individuals from nursing home settings to Assisted Living and Memory Care communities. While I have not had the chance to tour their other community in Elmore, MN I have seen their online virtual tour and have spoken to their nurse Amara on a couple of occasions- I was impressed by both. Always wonderful to work with! They explain the whole process in detail for you and your family to understand so you can trust the process! Carol Kreuser-Bade works so well with us and patients families so they are comfortable with the move and they understand the steps and what they all offer. Which leads me to my next rave! I have worked with Carol K.B. for about a year now. I met Carol through networking within the Mankato area. Carol has a tremendous heart for serving the elderly community and for sharing her knowledge. Carol spends a significant amount of time commuting between facilities to meet and discuss referral needs, ensuring all residents find a facility that is perfect for them. I enjoy working with Carol and would highly recommend Carol and Sunflower Communities for those very difficult situation or mental health needs.

The Sunflower Communities do a wonderful job in assisting people who need a place to call home. Carol Kreuser-Bade is amazing at promoting and educating potential residents about this facility. She is always a terrific resource to me, as I learn more about what the Sunflower Communities have to offer. She is a rock star!
