Community Neuro Rehab
A brain injury creates different challenges throughout the course of one's life. At Community NeuroRehab, we understand that needs change over time, and we do our best to diminish the cognitive, physical, behavioral, and psychosocial complications of brain injury. Our unique, community-based program is designed specifically for individuals experiencing a variety of difficulties as a result of a brain injury.

We take a person-centered approach to rehabilitation and offer compassion and respect to all those we serve.
Services
Many difficult treatment decisions have to be made by individuals experiencing brain injury, their families, case managers and other professionals. Community NeuroRehab's (CNR) referral specialists and admission team members provide education and support to these individuals, working to ensure they have accurate information to make informed decisions about their rehabilitation needs.
Community NeuroRehab offers hope to people recovering from a brain injury and provides support to their families and friends. We understand the changing needs of individuals at different stages in their recovery and have created a unique program, designed specifically for individuals experiencing a variety of difficulties as a result of a brain injury.
Treatment at our community-based residential rehabilitation centers focuses on working with participants as they become more independent with everyday activities of daily living and as they achieve greater freedom and independence. Recovery is maximized by providing a wide range of interventions designed to achieve individualized goals.
In many cases, the transition to home from residential treatment requires ongoing professional support to ensure success. To facilitate a durability of outcomes achieved, Community NeuroRehab's intermittent in-home rehabilitation focuses on working with participants as they are transitioning from our residential program to community living in their own homes.
CNR's program model is organized with the participant's individualized treatment team in mind and as such we do not employee doctors and therapists. We recognize that many participants may already have ancillary providers supporting them in the community. We encourage their continued support and active involvement on the participant's treatment team because we don't want to lose those relationships or their expertise.
