Forty years ago, groups of concerned citizens from the communities of Howard and Bryant (Miner-Hamlin Health Care Project) and Wessington Springs, Plankinton, and Woonsocket (Tri-County Health Care) formed their respective Federally Qualified Community Health Centers (FQHC) to ensure healthcare would remain in their communities.

Horizon behavioral health services provide support for individuals, groups, and families at all of our community health centers through telemedicine and onsite services.
Horizon dental clinics provide dental services and support for individuals and families. Many Americans - young or old, male or female, rich or poor - suffer from oral disease. Left untreated, oral disease causes pain, lowers productivity and increases our risks for other diseases. Horizon's dental offices allow everyone access to quality, affordable dental health care.

I've been Dr. Ruth Lim's patient for three years now after moving from 'another Huron Clinic who's Professional Competency and Knowledge
of my own Medical History proved to be disappointing'. Dr. Lim listened to me; that's a key factor for my choice of Primary Care Physician. Excellent
discussion and referral to Medical Specialists has also showed Dr. Lim's
dedication to my healthcare. Horizon's James Valley Community Health Center in Huron fits my needs due to: 'very well managed patient intake and professional front office staff too'.

I think it's great to have a clinic in town that offers a sliding fee. I'm very satisfied with my main doctor, as he is always pleasant, takes the time to understand my concerns, explains things thoroughly, and makes me feel comfortable. I also thinks it's wonderful that the clinic has partnered up with the Women's Wellness Center. I've had a chance to see a doctor from the WWC at the clinic. I have to say that I felt like my recent visit with the doctor from the WWC was less than satisfactory. I didn't feel as if she took my concerns seriously. It would be appreciated if she was more sensitive

Staff are friendly, multilingual, and available to answer questions and concerns you may have.
The provider I currently see after an emergency hospitalization follows up with thorough care and attentive monitoring.
The physician the hospital gave me, employed by another local clinic is uncomfortable, cold, disrespectful and outright uncaring about my particular case.
The provider at Horizon was a breathe of fresh air. I would expect that the other providers are just as great at treating
