Access the primary care services you need through a flat-rate monthly membership! Services covered through membership include wellness and health optimization, screening and prevention, acute visits, behavioral and mental health, women's health, and chronic disease management. Some specialty services and procedures can be added for a small additional fee.
As a patient of Fountain Square Holistic Family Medicine, we aim to make your access to medical care and communication with Dr. Koers seamless. Call 911 if you have a life threatening emergency. Call the practice regarding urgent medical concerns (need for a same day visit, deciding whether to go to ED, etc.).
