To familiarize you with our practice and simplify your registration, we have prepared this introductory information for you. We want our financial relationship to be comfortable and convenient for you. We intend for your experience with us to be as pleasant as possible. Bring your insurance card to each visit so that your dental care can be accurately reported.
Your copayment or 20% is required on the date of service and payment in full is due 30 days from the date of service. Payments can be made by mail or by calling our office. A 5% discount is offered for payment in full on or before the date of service if paid by cash, check, debit card, HSA card or FSA card.
