This place was out dated and dark and dirty. The room window shade would covered up the air conditioner if you pulled it down. I moved the recliner in the room and found what looked like old bacon strips balled up!!! The bed mattress was hard as a rock and lumpy and the controls for raising or lowering the bed would still have the bed moving after you released the button. The call for help button was not working so my mom was without help all night long!!! Unacceptable!!! The Administrator said it was the maker of the call button fault! Take responsibility for your staff and out dated equipment

Was the absolute worst Care Facility that I have ever seen. I was there before Xmas of 2020, contacted COVID-19, had two different patients crawl into my my private room, one looking for a bathroom, the other, his car keys, both times it took close to 15 minutes for any help was received. The food was horrid, cold and soggy being the only thing consistent. Totally inept and understaffed.

Treats everyone who walks through the door like family. Food is amazing, fun activities are held daily and the new therapy room is built perfectly with brand new equipment. Therapy staff makes getting stronger fun and is willing to work with anyone in the community. I would recommend this facility to anyone looking for long term care or rehab.

I would like to Thank all of the staff at Fort Dodge Health and Rehab for another amazing Halloween “Trick or Treat” night this year. You all went above and beyond to entertain your residents, their families and members of the community. The decorations were amazing, the games were fun and the kids loved the activities. A lot of work went into this and I wanted to let you know how appreciative me and my family are. Props to everyone who helped decorate, set up the games, make the food, get the prizes and candy for the kids, make sure all residents had plenty of candy to hand out to the kids

Ft Dodge Nursing and Rehab has friendly -helpful staff. The residents are well cared for and staff give residents loving and dedicated care. From my experience many enjoyable activities are provided and resident food choices are taken into consideration at resident requests. It would be a home well worth considering if one has a loved one needing placement.

I have been working in the Nursing Home field for over 20 years, and Fort Dodge Health and Rehab is by far the best I have ever worked in. The staff is the most competent, compassionate, caring people I have ever met and have the pleasure of working with. The atmosphere is always upbeat, the building is beautiful and family friendly.
