Independence Home Care of New York Inc. (IHC of NY), offers medical-related services to patients in a home setting. Home Health Care Services include medical or psychological assessment, wound care, medication teaching, pain management, disease education and management, and nursing care.
Services are provided by Care Coordinators- Licensed Practical Nurse, (CCLPN) with the support of Home Healthcare Aides (HHA'S) who carry out such task as; assistance with, Activities of Daily Living (ADL) including bathing, dressing, transferring, using the toilet, feeding, and walking, that reflect patients 'capacity for self-care.

CCLPN (Care Coordinator Licensed Practical Nurse): Provides services to individuals in their own homes and communities, who need assistance with their activities of daily living as a result of sickness, old age, disability and/or other inflictions. Experience in medical oversight and monitoring regarding to UTI's, Diabetes, Parkinson's, COPD and other medical conditions for the elderly is required.
