Services
Rocky Mountain Care has provided care since 1990. As a member of your community, our mission is to deliver compassionate hospice, home care, and skilled nursing services to all those entrusted in our care. Through leadership and trust, we function as a cohesive team dedicated to cost effective, high-quality outcomes for our residents.
Rocky Mountain Care does not exclude, deny benefits to, or otherwise discriminate against any person on the basis of race, color, national origin, disability or age in admission to, participation in, or receipt of the services and benefits under any of its programs and activities. Rocky Mountain Care does provide financial assistance of charity care based on available resources for those who cannot afford care.
Living more independently. These are the goals of Rocky Mountain Home Care. Through our staff of skilled and compassionate people, it is our mission to provide quality, individualized services which strengthen the independent lifestyle and promote dignity of the elderly and disabled in their own place of residence.
Hospice care assists individuals, their families and/or caregivers to achieve the best quality of life during a life-limiting illness. Hospice services include physical, emotional and spiritual care. Hospice Care focuses on patient comfort instead of continued treatment of the life-limiting condition.
Personal care is for patients who are in need of additional assistance in their own homes. It does not replace home health care or hospice care, but can be used in concert with these services. Personal care takes place in the home, but can also be used to supplement care in other locations.

Anyone over 18 years of age can qualify for personal care as long as there is an ability to pay and the services do not exceed the definitions of personal care. All Rocky Mountain Personal Care givers go through extensive background checks, TB testing, Health screening, and continue to receive additional patient care training through the year.
